摘 要:目的分析实习护生院前急救电话指导能力现状及其影响因素,并了解培训需求。方法采用方便抽样选取402例在福建省3所医院的实习护生采用院前急救电话指导能力问卷、临床沟通能力测评量表、批判性思维能力测评量表和培训需求问卷实施问卷调查。结果院前急救电话指导能力总分为(64.37±8.17)分。实习护生院前急救电话指导能力与临床沟通、共情、批判性思维均呈正相关(r=0.333,0.615,0.508,p<0.05)。多重逐步回归分析显示,确认病人问题、自信心、开放思想、系统化能力、分析能力、怜悯关怀、观点采择等7个因子是影响实习护生院前急救电话指导能力的主要因素。实习护生的培训内容需求较高的是常见急症院前急救技能(88.06%)和急救远程指导沟通技巧(86.57%),培训方式需求较高的是护士现场带教(95.52%)和情境剧模拟表演教学(85.57%),培训时间需求较高的是急诊科实习时培训(61.44%)。结论实习护生院前急救电话指导能力处于中等水平,受临床沟通能力、批判性思维能力、共情较低影响,需选择有针对性的培训方式进行干预。Objective To analyze the current situation of pre hospital emergency telephone guidance ability of nursing students to exploreits influencing factors and the training needs of this ability.Method 402 nursing students in 3 hospitals in Fujian Province were selected by convenient sampling and investigated by pre hospital emergency telephone guidance ability questionnaire,clinical communication competence scale,the Chinese critical thinking disposition inventory and the training demand questionnaire.Result The total score of telephone guidance ability was(64.37±8.17).The ability of telephone guidance of nursing students was positively correlated with clinical communication,empathy and critical thinking(r=0.333,0.615,0.508,p<0.05).Multiple stepwise regression analysis showed that confirming patient problems,self-confidence,open mind,systematic ability,analytical ability,compassion and care,and viewpoint selection were the main factors affecting the ability of telephone guidance of nursing students.The higher demand for training content of nursing students was common emergency pre hospital first aid skills(88.06%)and first aid guidance and communication skills(86.57%),the higher demand for training methods was nurse on-site teaching(95.52%)and situational drama simulation teaching(85.57%),and the higher demand for training time was emergency department internship training(61.44%).Conclusion The ability of telephone guidance of nursing students was at a medium level.Affected by the low clinical communication,critical thinking and empathy,targeted training methods should be selected for intervention.
