摘　　要：目的:探讨急性早幼粒细胞白血病(acute promyelocytic leukemia,APL)患者初发时骨髓内CD105标记IOD值(CD105-IOD)、CD105标记微血管密度(CD105-MVD)与外周血白细胞数量的关系。方法:收集我中心2019年01月至2020年06月初发APL患者共39例,根据中国APL诊疗指南将初发APL患者依照外周血白细胞计数分为高危组11例和中低危组28例,对其骨髓石蜡包埋标本切片行CD105免疫组化染色,计数CD105-MVD;显微摄影后应用image-pro plus软件进行图像分析并计算CD105-IOD。结果:初发APL的中低危组患者CD105-IOD、CD105-MVD显著高于高危组患者(30715.66±38817.03 vs 12271.36±14007.03,P=0.036;12.79±8.67 vs 7.55±3.76,P=0.012),Spearman相关性分析表明CD105-MVD与外周血白细胞计数增高存在显著相关性(ρ=-0.391,P=0.014)。结论:APL初发时高危组患者骨髓中CD105表达下降是引起其外周血白细胞计数增高的因素之一。Objective:To investigate the relationship between CD105-IOD,CD105-MVD and the count of peripheral blood leukocytes in patients with acute promyelocytic leukemia(APL).Methods:According to the Chinese guidelines for APL diagnosis and treatment,patients with newly diagnosed APL were divided into high-risk group and mid/low-risk group according to the number of peripheral blood leukocytes,CD105-MVD was counted after immunohistochemical staining of CD105 in paraffin embedded bone marrow of the above patients,and image-pro plus software was used for image analysis and CD105-IOD was calculated.Results:CD105-IOD and CD105-MVD in high-risk group were significantly lower than those in mid/low-risk group(30715.66±38817.03 vs 12271.36±14007.03,P=0.036;12.79±8.67 vs 7.55±3.76,P=0.012).Spearman correlation analysis showed that CD105-MVD was significantly correlated with leukocyte count(ρ=-0.391,P=0.014).Conclusion:The decrease of CD105 expression in bone marrow of high-risk group at the onset of APL is one of the factors causing the increase of peripheral blood leukocyte count.
