摘 要:目的 探讨PAX 1基因甲基化定量检测在宫颈癌及癌前病变诊断中的应用价值。方法 100例高危型人乳头状瘤病毒(HPV)感染者为研究对象,依据病理分级不同及国际妇产科联盟(FIGO)对宫颈癌的临床分期准则分为正常宫颈组(25例)、低级别病变组(30例)、高级别病变组(35例)及癌变组(10例)。对四组患者收集宫颈组织标本、宫颈刮片,用反转录聚合酶链式反应(PCR)及焦磷酸测序技术定量分析PAX 1基因9个点甲基化水平。对比四组病变组织中HPV阳性率与PAX1基因甲基化发生率,宫颈刮片和宫颈组织中PAX1基因甲基化水平。结果 正常宫颈组、低级别病变组、高级别病变组、癌变组HPV阳性率分别为36.0%、80.0%、91.4%、100.0%,PAX1基因甲基化发生率分别为0、6.7%、40.0%、100.0%,四组对比,差异均有统计学意义(P<0.05)。四组宫颈刮片和宫颈组织中PAX1基因甲基化水平对比,差异均有统计学意义(P<0.05)。癌变组宫颈刮片和宫颈组织中PAX1基因甲基化水平均明显高于正常宫颈组、低级别病变组、高级别病变组,差异均有统计学意义(P<0.05)。结论 在诊断高级别宫颈癌前病变方面定量检测宫颈组织内PAX1基因甲基化水平有效,且表达程度与疾病严重程度有关。Objective To discuss the application value of quantitative detection of PAX 1 gene methylation in the diagnosis of cervical cancer and precancerous lesions.Methods A total of 100 cases of high-risk human papillomavirus(HPV) infection were selected as the subjects of this study.According to the different pathological grades and the clinical staging guidelines of the International Federation of Gynecology and Obstetrics(FIGO) for cervical cancer,they were divided into normal cervical group(25 cases),low-grade lesion group(30 cases),high-grade lesion group(35 cases) and canceration group(10 cases).Cervical tissue specimens and cervical smears were collected from patients in four groups,and the methylation levels of PAX1 gene at 9 sites were quantitatively analyzed by polymerase chain reaction(PCR) and pyrosequencing.Comparison was made on HPV positive rate and PAX1 gene methylation rate in the lesion tissue,and the methylation level of PAX1 gene in cervical smear and cervical tissue in four groups.Results The positive rates of HPV in the normal cervical group,low-grade lesion group,high-grade lesion group,and canceration group were 36.0%,80.0%,91.4%,and 100.0%,respectively,and the rates of PAX1 gene methylation were 0,6.7%,40.0%,100.0%,and the difference was statistically significant among the four groups(P<0.05).The methylation levels of PAX1 gene in cervical smears and cervical tissue of the four groups were compared,and the differences were statistically significant(P<0.05).The methylation level of PAX1 gene in cervical smear and cervical tissue in the canceration group was significantly higher than that in the normal cervical group,the low-grade lesion group and the high-grade lesion group,and the differences were statistically significant(P<0.05).Conclusion In the diagnosis of high-grade cervical precancerous lesions,quantitative detection of PAX1 gene methylation level in cervical tissue is effective,and the expression level is associated with the severity of the disease.
