摘  要:目的 以人群为基础,了解山西省消化性溃疡患病状况,探究胃溃疡和十二指肠溃疡发病的影响因素,为疾病防控策略的制定提供依据。方法 采用多阶段分层随机抽样的方法,在全省4个区/县选取18~64岁常住居民通过问卷调查、13C呼气试验和胃镜检查对消化性溃疡的流行状况及危险因素开展调查;采用单因素分析和多因素Logistics回归分析,确定相应的危险因素。结果 3 456名调查对象中,共有胃溃疡131名,患病率为3.79%;十二指肠溃疡52名,患病率为1.50%。男性、工作性质中兼有体力劳动和脑力劳动、吸烟、饮酒和幽门螺杆菌感染和不良情绪是胃溃疡发病的危险因素(OR>1);吸烟、饮酒和不良情绪是十二指肠溃疡发病的危险因素(OR>1)。结论 预防和控制消化性溃疡发病的措施包括从已确诊患者、疑似症状或家庭成员确认感染者、普通公众三个角度开展幽门螺杆菌的防控、加强吸烟、饮酒的健康教育力度、关注患者心理健康,改善家庭关系等。Objective To investigate the disease status and risk factors of peptic ulcers in Shanxi Province and provide the basis for the formulation of disease prevention and control strategies based on population.Methods Multi-stage stratified random sampling was used to investigate the prevalence and risk factors of peptic ulcers among permanent residents aged 18~64 from 4 districts or counties of the province through an inquiry survey,13 C breath test and gastroscopy.Single-factor analysis and multivariate Logistics regression analysis were used to determine the corresponding risk factors.Results Among the 3 456 subjects,131 had gastric ulcers,and the prevalence rate was 3.79%.52 patients had duodenal ulcers,and the prevalence rate was 1.50%.Male,physical and mental work,smok-ing,drinking alcohol,Helicobacter pylori infection and bad mood are risk factors for gastric ulcers(OR>1).Smoking,drinking alcohol and bad mood are risk factors for the onset of duodenal ulcer(OR>1).Conclusion Measures,including the prevention and control of Helicobacter pylori from the perspectives of confirmed patients,suspected symptoms or confirmed infections by family members,and the general public,strengthening health education on smoking and drinking,and paying attention to the psychology of patients′ health,improving family relations,etc,can prevent and control the onset of peptic ulcers.
