摘 要:目的应用切开植入法制备乳腺癌转移淋巴结模型,并用二维超声,彩色多普勒超声、超声弹性成像及超声造影评价移植瘤及转移淋巴结生长特征。方法取雄性新西兰大白兔5只,采取切开植入法及穿刺针植入法分别将VX2肿瘤组织植入同一只大白兔左、右下肢肌肉丰富处制备荷瘤兔,比较该两种方法肿瘤生长情况。再选取雌性新西兰大白兔15只作为实验兔,采用穿刺针植入法及切开植入法中成功率更高的植入法,将荷瘤兔肿瘤组织移入实验兔左侧胸壁第3乳头乳垫内,常见超声技术观察原位乳腺癌及转移淋巴结生长情况,约6周取出乳腺癌及淋巴结病检。结果3周左右荷瘤兔下肢可触及肿瘤,切开植入法接种成功率为100%(5/5),穿刺针植入法接种成功率为60%(3/5)。实验兔移植后1~2周可触及肿瘤组织块,成瘤率为93.3%(14/15),超声显示左侧胸壁第3乳头处类圆形低回声结节,周边可见较丰富彩流信号,弹性成像示肿瘤较软,超声造影示肿瘤呈向心性不均匀增强,部分可见充盈缺损。约6周触及淋巴结,转移率为64.3%(9/14),超声显示腋窝可见类圆形或椭圆形低回声淋巴结,淋巴门结构消失或移位,周边可见点棒状彩流信号,弹性成像示淋巴结较软,超声造影示整体呈向心性不均匀增强。结论采用切开植入法成功制备乳腺癌转移淋巴结模型,成瘤率高,较易转移,各种常见超声技术能便捷的探查移植瘤及转移淋巴结的生长特征与变化过程,是较理想的乳腺癌转移淋巴结动物模型。Objective To establish a lymph node metastasis model of breast cancer by incision implantation and to evaluate its growth characteristics by two-dimensional ultrasound,color doppler ultrasound,ultrasound elastography and contrast-enhanced ultrasound.Methods Five male New Zealand white rabbits were selected.VX2 tumor tissue was implanted into the muscle rich area of the left lower limb of the white rabbit by incision implantation method,and VX2 tumor tissue was implanted into the muscle rich area of the right lower limb of the same white rabbit by puncture needle implantation method to prepare the tumor-bearing rabbits,and the tumor growth of the two methods was compared.Then,15 female New Zealand white rabbits were selected as experimental rabbits,and the implantation method with higher success rate was adopted in the puncture needle implantation method and the incision implantation method.The tumor tissue of the tumor-bearing rabbit was moved into the third nipple pad on the left chest wall of the experimental rabbit.The growth of in situ breast cancer and metastatic lymph nodes were observed by common ultrasound technology.Breast cancer and lymph nodes were removed at approximately 6 weeks for histopathologic examination.Results Tumors could be palpated in the lower limbs of tumor-bearing rabbits at about 3 weeks,and the success rate of incision implantation method was 100%(5/5)and puncture needle implantation method was 60%(3/5).Tumor tissue masses could be palpated in experimental rabbits 1-2 weeks after transplantation,with a tumorigenic rate of 93.3%(14/15).Ultrasound showed circular low-echo nodules at the third papillary of the left chest wall,with abundant color flow signals around,Elastomography showed the tumors were soft,contrast-enhanced ultrasonography showed that the tumors were inhomogeneous enhancement with partial filling defects.The lymph nodes were touched at about 6 weeks,and the metastasis rate was 64.3%(9/14).Ultrasound showed round or oval hypoechoic lymph nodes in the axilla,lymphatic hil
