食管手术术中留置营养管的方法研究

摘　　要：目的探索新的食管手术术中营养管置入方式,提高置管成功率,缩短手术时间。方法选择2018年9月至2021年8月新乡医学院第一附属医院行胸腹腔镜联合Mckeown食管切除术的患者共92例,按营养管置管方式分为对照组40例和观察组52例。对照组采用传统的经鼻营养管置入法,观察组采用经术野营养管置入法,对比两组手术时长、术后住院天数、住院费用、吻合口瘘、营养管放置是否成功等指标。结果两组在术后住院天数、住院费用、吻合口瘘方面差异无统计学意义(P>0.05)。与对照组相比,观察组手术耗时明显缩短,营养管置管成功率明显提高,差异有统计学意义(P<0.05)。结论经术野营养管置入法应用于食管手术,缩短了手术时间,提高了置管成功率,解决了传统经鼻置管的缺点,具有经济、实用、安全等优点,是一种值得推广的食管手术术中营养管置入方法。Objective To explore a new way of nutrient tube placement in esophageal surgery,improve the success rate of tube placement and shorten the operation time.Methods 92 patients who underwent thoracoscopic laparoscopic combined with McKeown esophagectomy in our hospital from September 2018 to August 2021 were selected,and divided into control group(40 cases)and observation group(52 cases)according to the way of nutrient tube placement.The control group was treated with traditional transnasal nutrition tube placement;the observation group was treated with nutrient tube placement through the operation field.The operation duration,postoperative hospitalization days,hospitalization expenses,anastomotic fistula and the success of nutrient tube placement were compared between 2 groups.Results There were no significant differences in postoperative hospitalization days,hospitalization expenses and anastomotic fistula between 2 groups(P>0.05).Compared with the control group,the operation time of the observation group was significantly shortened and the success rate of nutrition tube placement was significantly improved(P<0.05).Conclusion The application of nutrient tube placement through operation field in esophageal surgery can shorten the operation time,improve the success rate of nutrition tube placement,solve the disadvantages of traditional transnasal tube placement,and it has the advantages of economy,practicality and safety,thus this method is worth promoting.
