超声乳化白内障吸除术治疗硬核白内障的效果及对术后干眼症的影响

摘　　要：目的探讨超声乳化白内障吸除术治疗硬核白内障的临床效果及对术后干眼症的影响。方法选取2017年1月至2019年1月浙江中医药大学附属湖州市中医院收治的88例硬核白内障患者,采用随机数字表法分为对照组和观察组,每组44例。对照组使用小切口白内障切除术治疗,观察组使用超声乳化白内障吸除术进行治疗。比较两组术前及术后1d的眼压、散光度数、裸眼视力、泪腺功能(泪液分泌量、泪膜破裂时间)、干眼症评分、散光轴位,以及术后并发症的发生情况。结果术前两组患者的眼压、散光度数、裸眼视力、泪液分泌量、泪膜破裂时间、干眼症评分比较差异均无显著性(P>0.05);术后1d两组的眼压、散光度数、干眼症评分明显低于术前,且观察组明显低于对照组,裸眼视力、泪液分泌量、泪膜破裂时间明显高于术前,且观察组明显高于对照组,差异均有显著性(P<0.05)。手术前后两组散光轴位组内及组间比较差异均无显著性(P>0.05)。观察组术后并发症发生率为13.64%,明显低于对照组的31.82%,差异有显著性(P<0.05)。结论超声乳化白内障吸除术治疗硬核白内障的效果显著,安全性好,术后干眼症状轻,值得临床应用。Objective To study the effect of phacoemulsification cataract aspiration in the treatment of hard nuclear cataract and its influence on postoperative dry eye.Method A total of 88 patients with hard nuclear cataract admitted to Huzhou Hospital of Traditional Chinese Medicine Affiliated to Zhejiang University of Traditional Chinese Medicine from January 2017 to January 2019 were selected and randomly divided into a control group and an observation group,with 44 cases in each group.The control group was treated with small-incision cataract resection,and the observation group was treated with phacoemulsification cataract extraction.IOP,astigmatism,naked eye vision,lacrimal gland function(tear secretion,tear film rupture time),dry eye score,astigmatism axial position,and postoperative complications were compared between the two groups.Result There were no significant differences in intraocular pressure,astigmatism,uncorrected visual acuity,tear secretion,tear film breakup time and dry eye score between the two groups before surgery(P>0.05).On day 1 after surgery,iOP,astigmatism and dry eye score in both groups were significantly lower than those before surgery,and observation group was significantly lower than control group,naked eye vision,tear secretion and tear film rupture time were significantly higher than those before surgery,and observation group was significantly higher than control group,the differences were significant(P<0.05).There was no significant difference in the axial astigmatism between the two groups before and after surgery(P>0.05).The incidence of postoperative complications in the observation group was 13.64%,which was significantly lower than 31.82% in the control group,with a significant difference(P<0.05).Conclusion Phacoemulsification for cataract surgery has significant effect,good safety,and mild postoperative dry eye symptoms,which is worthy of clinical application.
