摘 要:目的评价Gore-Tex缝线四点式人工晶状体睫状沟固定术的临床疗效和安全性。方法回顾性分析行人工晶状体睫状沟缝线固定术的患者29例(30眼),其中20例(20眼)采用传统聚丙烯缝线人工晶状体睫状沟缝线固定术(对照组),9例(10眼)行Gore-Tex缝线四点式人工晶状体睫状沟缝线固定术(四点式组)。对术后裸眼视力、验光值、裂隙灯检查及眼压结果等进行统计分析,随访时间为12个月。结果四点式组于术后1个月开始裸眼视力明显提高(P<0.05),对照组于术后3个月开始裸眼视力明显提高(P<0.05)。术后1、3、6、12个月时,四点式组较术前裸眼视力提高量均优于对照组(P<0.05)。术后1周和1、3、6、12个月时,四点式组眼内残余散光值均小于对照组(P<0.05)。2组均未出现严重的眼部并发症。结论Gore-Tex缝线四点式人工晶状体睫状沟固定术通过四点固定,使术后人工晶状体不易发生旋转和偏斜,显著降低术后眼内残余散光,更早改善术后裸眼视力,手术安全有效。Objective To evaluate the efficacy and safety of four-port posterior chamber intraocular lens(IOL)sulcus fixation using Gore-Tex suture.Methods We retrospectively analyzed 29 patients(30 eyes)who underwent IOL sulcus fixation.Among them,20 patients(20 eyes)underwent the traditional surgery(control group)and 9 patients(10 eyes)underwent IOL sulcus fixation using Gore-Tex suture(four-port group).Data on the uncorrected visual acuity(UCVA),IOL position,intraocular pressure,and refractory status during the 12 months after the operation were collected and statistically analyzed.Results UCVA improvement started at 1 month after the operation in the four-port group(P=0.011)and at 3 months after the operation in the control group(P=0.038).At the 1-,3-,6-,and 12-month follow-up,the improvement of postoperative UCVA was better in the four-port group than in the control group(P<0.05).At 1 week and 1,3,6,and 12 months after the operation,residual astigmatism was lower in the four-port group than in the control group(P<0.05).No severe ocular complications occurred in both groups.Conclusion IOL sulcus fixation using Gore-Tex suture may reduce postoperative residual astigmatism and improve UCVA due to less IOL misalignment.
