摘　　要：目的:系统评价针刺治疗脑卒中后构音障碍的疗效及安全性,为临床应用提供依据。方法:检索中国知网、万方、维普、中国生物医学文献数据库、PubMed及Corchrane Library中所有针刺治疗脑卒中后构音障碍的临床随机对照试验,检索时间为建库起至2021年6月。经严谨筛选后,提取符合要求的文献数据及疗效指标,运用RevMan5.3软件进行Meta分析。结果:最终纳入文献20篇,病例1504例。其中18篇选用临床疗效进行评价,结果显示:OR=3.68,95%CI(2.74,4.96),P=0.001。5篇以改良Frenchay构音障碍量表(FDET)a项数作为结局指标,结果显示:SMD=0.95,95%CI(0.74,1.15),P=0.001。3篇以FDET总分作为结局指标,结果显示:MD=-8.68,95%CI(-12.31,-5.05),P=0.001。2篇以言语清晰度作为结局指标,结果显示:MD=6.25,95%CI(-3.07,15.57),P=0.190。结论:针刺结合常规言语治疗能显著改善脑卒中患者的构音功能,且安全性较高。但上述结论仍需要更大样本、更高质量的临床试验提供依据。Objective:To systematically evaluate therapeutic effect of acupuncture on post-stroke dysarthria and its safety,and provide a basis for clinical application.Methods:Clinical randomized controlled trials regarding acupuncture for post-stroke dysarthria were searched in CNKI,Wanfang,VIP,CBM,PubMed and Cochrane Library.The limit time was from the establishment to Jun 2021.After strict screening,literature data were extracted according to inclusion criteria and effect indexes.RevMan 5.3 was used to perform Meta-analysis.Results:A total of 20 literatures were finally enrolled,including 1504 cases.Among them,18 literatures used clinical therapeutic effect to assess,OR=3.68,95%CI(2.74,4.96),P=0.001;five literatures used item a of modified Frenchay dysarthria evaluation(FDET)as index,SMD=0.95,95%CI(0.74,1.15),P=0.001;three literatures used total FDET score as index,MD=-8.68,95%CI(-12.31,-5.05),P=0.001;two literatures used speech intelligibility as index,MD=6.25,95%CI(-3.07,15.57),P=0.190.Conclusion:Acupuncture combined routine speech therapy can significantly improve speech function of post-stroke dysarthria patients with high safety.However,above conclusions still need to be proved by higher-quality and larger-sample trials.
