摘  要:目的 探究粤西地区心脑血管疾病患者CYP2C19基因多态性分布情况。方法 选取2020年9月—2021年8月共790例心脑血管疾病患者为研究对象,抽取空腹静脉血,检测CYP2C19基因和位点突变情况。结果790例粤西地区心脑血管疾病患者CYP2C19*1、*2和*3的等位基因频率分别为63.04%、31.65%和5.32%;790例患者中CYP2C19*1/*1、*1/*2、*2/*2、*2/*3、*3/*3和*1/*3基因型的分布频率分别为38.86%、40.25%、10.00%、2.41%、0.38%和8.10%。不同性别心脑血管疾病患者的基因型分布间对比,差异无统计学意义(P>0.05);790例患者中快代谢型、中间代谢型和慢代谢型分别占比38.86%、48.35%和12.78%,不同性别患者基因代谢型分布中,男性分别为36.92%、49.28%、13.80%,女性分别为43.53%、46.12%、10.34%,差异无统计学意义(χ^(2)=3.020、0.656、1.754,P>0.05)。结论 粤西地区心脑血管疾病患者CYP2C19的等位基因频率分布以CYP2C19*1为主,基因型分布以CYP2C19*1/*1和CYP2C19*1/*2为主,代谢型则以中快代谢型和中间代谢型为主,不同性别心脑血管疾病患者中CYP2C19的等位基因频率分布和代谢型分布类似,临床应根据本地区心脑血管疾病患者的CYP2C19基因多态性分布情况进行个体化用药。Objective To investigate the distribution of CYP2C19 gene polymorphisms in patients with cardiovascular and cerebrovascular diseases in western Guangdong.MethodsA total of 790 patients with cardiovascular and cerebrovascular diseases from September 2020 to August 2021 were selected as the research objects,and fasting venous blood was drawn to detect CYP2C19 gene and locus mutation.ResultsThe allele frequencies of CYP2C19*1,*2 and *3 in 790 patients with cardiovascular and cerebrovascular diseases in western Guangdong were 63.04%,31.65% and 5.32%,respectively.The distribution frequencies of CYP2C19*1/*1,*1/*2,*2/*2,*2/*3,*3/*3 and *1/*3 genotypes in 790 patients were 38.86%,40.25%,10.00%,2.41%,0.38% and 8.10%.There was no statistically significant difference in the distribution of genotypes among patients with cardiovascular and cerebrovascular diseases of different genders(P>0.05).Among the 790 patients,fast metabolizer,intermediate metabolizer and slow metabolizer accounted for 38.86%,48.35% and 12.78%,respectively.In the genotype distribution of patients with different genders,males were 36.92%,49.28%,and 13.80%,and females were 43.53%,46.12%,and 10.34%,respectively,and the difference was not statistically significant(χ~2=3.020,0.656,1.754,P>0.05).ConclusionThe allele frequency distribution of CYP2C19 in patients with cardiovascular and cerebrovascular diseases in western Guangdong is mainly CYP2C19*1,and the genotype distribution is mainly CYP2C19*1/*1 and CYP2C19*1/*2.The metabolotypes were medium fast metabolizers and intermediate metabolizers.The allele frequency distribution of CYP2C19 in patients with cardiovascular and cerebrovascular diseases of different genders was similar to that of metabotypes.Clinically,individualized medication should be performed according to the distribution of CYP2C19 gene polymorphisms in patients with cardiovascular and cerebrovascular diseases in the region.
