小切口非超声乳化白内障手术治疗硬核白内障的效果及对角膜改善情况、泪膜功能、前房房水炎症指标的影响

摘　　要：目的分析小切口非超声乳化白内障手术治疗硬核白内障的效果及安全性。方法选择我院2019年1月至2021年1月收治的86例硬核白内障患者为研究对象,随机将其分为对照组(n=43,传统超声乳化手术)与研究组(n=43,小切口非超声乳化白内障手术)。比较两组的角膜改善情况、泪膜功能、前房房水炎症指标以及不良反应发生情况。结果手术治疗3个月后,研究组的角膜内皮细胞密度、细胞平均面积、角膜厚度、泪膜破裂时间(BUT)、主观干眼症状问卷积分(SDES)、泪液分泌试验(SⅠT)优于对照组(P<0.05)。手术治疗1个月后,两组的闪光、细胞计数均增加,但研究组小于对照组(P<0.05)。研究组的不良反应总发生率低于对照组(P<0.05)。结论小切口非超声乳化白内障手术治疗硬核白内障的效果良好,利于改善患者的角膜细胞,增强泪膜的稳定性,减轻前房房水炎症,减少不良反应。Objective To analyze the effect and safety of small incision non-phacoemulsification cataract surgery in the treatment of hard nuclear cataract. Methods A total of 86 patients with hard nuclear cataract admitted in our hospital from January 2019 to January 2021 were selected as the research objects. The patients were randomly divided into control group(n=43, traditional phacoemulsification) and study group(n =43, small incision non-phacoemulsification cataract surgery).The corneal improvement, tear film function, inflammation indexes of anterior chamber aqueous humor and adverse reactions were compared between the two groups. Results After 3 months of surgical treatment, the corneal endothelial cell density, average cell area, corneal thickness, tear break-up time(BUT), subjective symptoms of dry eye questionnaire scores(SDES) and schirmer Ⅰ test(SⅠT) in the study group were better than those in the control group(P<0.05). After 1months of surgical treatment, the flash and cell counts of the two groups increased, but those in the study group were less than the control group(P <0.05). The total incidence of adverse reactions in the study group was lower than that in the control group(P<0.05). Conclusion Small incision non-phacoemulsification cataract surgery for hard nuclear cataract has a good effect, which is beneficial to improve the patients’ corneal cells, enhance the stability of the tear film, relieve the inflammation of anterior chamber aqueous humor, and reduce adverse reactions.
