某院2020年门诊不合理处方点评分析

摘　　要：目的统计分析厦门市中医院2020年门诊处方点评情况,以期为前置审方的实施提供数据参考。方法抽查厦门市中医院2020年门诊处方1484张,根据《医院处方点评管理规范(试行)》《抗菌药物临床应用指导原则(2015年版)》《处方管理办法》及药品说明书、各科用药指南等参考资料进行处方审核,对不合理处方进行点评分析。结果抽取2020年门诊处方1484张,不合理处方112张(7.55%),其中5张有2处不合理,共计不合理处方117张。在所有不合理处方中,不规范处方16张(13.68%)、用药不适宜处方96张(82.05%)、超常处方5张(4.27%);不合理处方涉及40个科室,分布较散,排名前三的科室分别为儿科、骨三科、消化内科。结论厦门市中医院门诊处方合格率仍有待提高,医院应及时开展前置审方并持续改进与规范临床合理用药。Objective To analyse comments of outpatient prescription of Xiamen Hospital of TCM in 2020,in order to provide data reference for the implementation of pre-prescription review.Methods In 2020,1484 outpatient prescriptions were spot-checked of Xiamen Hospital of TCM in 2020,which were commented by“Hospital Prescription Review Management Standard(Trial)”“Guiding Principles of Clinical Application of Antibacterials(2015 Edition)”“Measures for the administration of prescriptions”and drug instructions,guidelines for the use of drugs in various subjects,comment and analyze unreasonable prescriptions.Results Among the 1484 outpatient prescriptions sampled in 2020,112 prescriptions(7.55%)were unreasonable,of which,5 unreasonable prescriptions had 2 irrational ones,and a total of 117 prescriptions of unreasonable prescriptions.Of all the unreasonable prescriptions,there were 16 prescriptions(13.68%)of non-standard,96 prescriptions(82.05%)of inappropriate grug use and 5 prescriptions(4.27%)of abnormal.Unreasonable prescriptions involved 40 departments with scattered distribution,and the top three departments were Pediatrics,Orthopedics and Gastroenterology.Conclusion The qualified rate of outpatient prescriptions in Xiamen Hospital of TCM still need to be improved.The hospital should carry out pre-trial prescription in time and continuously improve and standardize clinical rational drug use.
