丹参注射液联合阿托伐他汀钙片治疗缺血性脑卒中的临床疗效及其对患者颈动脉斑块面积的影响

摘　　要：目的观察丹参注射液联合阿托伐他汀钙片治疗缺血性脑卒中的临床疗效及其对患者颈动脉斑块面积的影响。方法选取2020年1月—2021年2月长沙市中心医院收治的缺血性脑卒中患者90例,采用随机数字表法分为对照组与观察组,各45例。对照组予以阿托伐他汀钙片,观察组在对照组治疗基础上加用丹参注射液,2组疗程均为1个月。比较2组临床疗效、改良Rankin量表(MRS)评分、改良Barthel指数(MBI)评分、颈动脉斑块面积、血液流变学指标[全血黏度、纤维蛋白原(Fig)、血细胞比容(HCT)]、不良反应总发生率。结果观察组总有效率为93.33%,高于对照组的77.78%(χ^(2)=4.406,P=0.036)。治疗后,观察组MRS评分低于对照组,MBI评分高于对照组,颈动脉斑块面积小于对照组(P<0.01)。治疗后,观察组全血黏度、Fig、HCT低于对照组(P<0.05)。观察组不良反应总发生率为11.11%,与对照组的8.89%比较,差异无统计学意义(P=1.000)。结论丹参注射液联合阿托伐他汀钙片治疗缺血性脑卒中的临床疗效确切,可促进功能恢复,提升患者生活自理能力,缩小颈动脉斑块面积,稳定血液流变学,且安全性较高。Objective To observe the clinical effect of Danshen injection combined with atorvastatin calcium tablets on treating ischemic stroke and its influence on carotid plaque area of patients.Methods A total of 90 cases of ischemic stroke patients were selected from January 2020 to February 2021 in Changsha Central Hospital,which were divided into control group and observation group according to the random number table method,45 cases in each group.The control group was given atorvastatin calcium tablets,while the observation group was given Danshen injection on the basis of the control group.The treatment course of the two groups were one month.Clinical efficacy,modified Rankin Scale(MRS)score,modified Barthel Index(MBI)score,carotid plaque area,hemorheological indexes[whole blood viscosity,fibrinogen(Fig),hematocrit(HCT)],incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 93.33%,higher than 77.78%in the control group(χ^(2)=4.406,P=0.036).After treatment,the MRS score of observation group was lower than that of control group,MBI score was higher than that of control group,and carotid plaque area was smaller than that of control group(P<0.01).After treatment,the whole blood viscosity,Fig and HCT in the observation group were lower than those in the control group(P<0.05).The total incidence of adverse reactions in the observation group was 11.11%,compared with 8.89%in the control group,the difference was not statistically significant(P=1.000).Conclusion Danshen injection combined with atorvastatin calcium tablets have an exact clinical effect on treating ischemic stroke,which can promote functional recovery,improve patients′self-care ability,reduce the area of carotid plaque,stablilize hemorheology and with high safety.
