食管-胃底套扎术联合艾司奥美拉唑镁肠溶片治疗胃食管反流病的远期效果观察

摘　　要：目的分析胃镜下食管-胃底黏膜套扎术联合艾司奥美拉唑镁肠溶片治疗胃食管反流病(GERD)的远期效果。方法收集2019年1月至2020年1月郑州大学第一附属医院门诊及住院的符合条件的GERD患者的临床资料,以随机数字表法分组法分为对照组与观察组,每组80例。对照组给予艾司奥美拉唑镁肠溶片口服,观察组在口服艾司奥美拉唑镁肠溶片基础上接受食管-胃底黏膜套扎术。观察两组治疗前后食管24 h pH检测结果(24 h pH<4中位时间百分比、De Messter评分)、比较GERD-HRQL评分及,分析联合食管-胃底黏膜套扎术前后相关指标变化,统计并发症情况。结果两组治疗后6个月、12个月时24 h pH<4中位时间百分比及De Messter评分、GERD-HRQL评分均显著低于治疗前,且观察组治疗后各指标均优于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为2.5%(2/80)。结论胃镜下食管-胃底黏膜套扎术与艾司奥美拉唑镁肠溶片联合治疗胃食管反流病效果优于单口服艾司奥美拉唑镁肠溶片,可有效且长时间控制反流症状,减少Barrett发生,缩短用药时间,提高生存质量,安全可靠。Objective To analyze the long-term efficacy of endoscopic esophageal-gastric Mucosal ligation combined with Esomeprazole and magnesium enteric-coated tablets on acid reflux disease.Methods The clinical data of eligible GERD patients in the outpatient and inpatient department of the First Affiliated Hospital of Zhengzhou University from January 2019 to January 2020 were collected.They were randomly divided into control group and observation group,with 80 cases in each group.The control group was given oral esomeprazole magnesium enteric coated tablets,and the observation group received esophageal gastric fundus mucosal ligation on the basis of oral esomeprazole magnesium enteric coated tablets.The 24 h esophageal pH test results(24 h pH<4 median time percentage,Demester score),GERD-HRQL score were observed before and after treatment,the changes of relevant indexes before and after combined esophageal gastric fundus mucosal ligation were analyzed,and the complications were counted.Results The percentage of 24 h pH<4 median time,Demester score and GERD-HRQL score of the two groups at 6 and 12 months after treatment were significantly lower than those before treatment,and the indexes of the observation group were better than those of the control group after treatment(P<0.05).The incidence of complications in the observation group was 2.5%(2/80).Conclusions The effect of gastroscopic esophageal gastric fundus mucosal ligation combined with esomeprazole magnesium enteric coated tablets on gastroesophageal reflux disease is better than that of oral esomeprazole magnesium enteric coated tablets alone.It can effectively and for a long time control reflux symptoms,reduce Barrett,shorten medication time,improve quality of life,and is safe and reliable.
