摘　　要：目的:评价自体血小板浓缩物在重度牙周炎拔牙后位点保存中的应用效果。方法:收集我院口腔科因重度牙周炎需拔除并日后需要进行延期种植修复的患者54例(57颗患牙),分为CGF组18例(20颗患牙)、PRF组18例(19颗患牙)及未进行位点保存的空白对照组18例(18颗患牙)。对各组拔牙窝拔牙术后即刻(0天)、7天、14天愈合面积差值、术前与术后6个月牙槽嵴高度差值、牙槽骨宽度差值、颊侧角化龈宽度(width of keratinized tissue,KTW)、牙龈退缩(gingival recession,GR)以及种植体负重1年后的存留率进行比较。结果:术后即刻各组的创口面积无统计学差异,术后7天、14天,CGF组拔牙创口愈合面积最大,空白对照组创口愈合面积最小。拔牙术后6个月,CGF组与PRF组间△H、△W的比较无统计学差异,且均优于空白对照组。拔牙术前的KTW、GR各组之间比较无统计学差异。拔牙术后6个月,各组KTW较术前均存在统计学差异。CGF组、PRF组拔牙术后6个月的KTW高于对照组,差异有统计学意义,但CGF组和PRF组间KTW无统计学差异。CGF组、PRF组术后6个月GR值与术前无统计学差异,对照组与术前存在统计学差异,术后6个月CGF组与PRF组GR值无统计学差异,且均低于对照组。种植体负重1年后,CGF组、PRF组存留率为100%,对照组存留率为94.44%;三组间存留率无统计学差异。结论:CGF与PRF均对于重度牙周炎拔牙后的位点保存有一定的维持作用,可以减少拔牙术后的牙槽骨吸收。CGF创口愈合效果更优于PRF,但长期临床效果仍需进一步观察。Objective:To evaluate the application effect of platelet concentrates in alveolar ridge preservation on severe periodontitis after tooth extraction.Methods:A total of 54 patients(57 diseased teeth)diagnosed as severe periodontitis which need to be implanted at our institution were included.The patients were divided into three groups,the experiment groups(PRF group and CGF group)and the control group(simple blood clot group).The patients were followed up for 0,7 and 14 days after surgery to observe the soft tissue healing area.Before operation and 6 months after operation,the KTW and GR were examined.The change of KTW and GR in the three groups were compared.The survival rate were compared in the three groups 1-year post-load.Results:There was no statistical significance in the three groups on the soft tissue healing area after tooth extraction.The soft tissue healing area of the CGF group were the maximum while that of the control group were the minimum 7 and 14 days after surgery.There was no statistical significance between the CGF group and the PRF group on△H and△W.△H and△W of the two experiment groups were both better than that of control group 6 months after operation.There was no statistical difference in the three groups on KTW and GR before operation.KTW of the two experiment groups were both higher than that of the control group 6 months after operation,but there was no statistical difference between the group of CGF and the group of PRF on KTW.There was no significant difference between 6 months after operation and before operation on GR both in the PRF group and the CGF group.There was significant difference between 6 months after operation and before operation on GR in the control group.There was no significant difference between the CGF group and the PRF group on GR 6months after operation,and GR of the two experiment groups were both lower than that of the control group.The survival rate were both 100%in the two experiment groups,and was 94.44%in the control group.There was no significant di
