退药制度对住院药房退药情况改进效果的分析与探讨

摘　　要：目的:分析德阳市人民医院退药管理制度实施前后住院病区退药情况,为制度的优化提出建议。方法:收集退药管理制度建立前后,住院药房2018年12月—2019年2月及2019年12月—2020年2月两段时间区间的同比退药数据,分析退药次数、退药科室、退药品种、退药原因等的改善情况,提出改进措施。结果:经过退药制度的建立和实施,12月份退药次数同比减少3862次,改善率为45.94%;1月份同比减少2706次,改善率为32.31%;2月份同比减少1813次,改善率为28.09%;涉及退药的科室中胃肠外科改善最佳,改善率为77.66%,其次为肿瘤科(67.27%)、肝胆胰外科(63.88%);退药品种及退药原因整改前后相似。结论:通过退药管理制度的实施,全院退药情况得到一定改善,但仍需通过针对性考核、强化合理用药审核、加强临床宣教等措施进一步优化。Objective:To analyze the situation of drug withdrawal in inpatient ward before and after the implementation of Drug Withdrawal Management System in hospital,and to put forward suggestions for the optimization of the system.Methods:The year-on-year drug withdrawal data of inpatient pharmacies from December 2018 to February 2019 and from December 2019 to February 2020 before and after the establishment of Drug Withdrawal Management System in hospital were collected to analyze the improvement situation of drug withdrawal times,drug withdrawal departments,drug types,drug withdrawal reasons,etc.,and put forward improvement measures.Results:After the establishment and implementation of the drug return system,the number of retrograde drugs in December(reduced 3862 times and the improvement rate 45.94%),January(reduced 2706 times and the improvement rate 32.31%),and February(reduced 1813 times and the improvement rate 28.09%)were compared with the same period last year.Among the departments involved in drug withdrawal,the improvement rate of gastrointestinal surgery was 77.66%,followed by oncology department(67.27%)and hepatopancreatobiliary surgery department(63.88%).The types and reasons of drug withdrawal were similar before and after rectification.Conclusion:Through the implementation of the drug withdrawal management system,the situation of drug withdrawal in our hospital has been improved to a certain extent,but it still needs to be further controlled through targeted assessment,strengthening rational drug use review,and strengthening clinical education.
