摘　　要：目的观察Ki-67及髓母细胞瘤(MB)四种分子分型对MB患者预后的危险分级。方法选取2009-01—2018-01在郑州大学第一附属医院手术且术后病理报告为髓母细胞瘤的92例患者为研究对象,收集、整理其临床资料和生存资料。运用Kaplan-Meier法计算分析总生存期(OS)及无进展生存期(PFS)的生存率,运用Log-Rank检验比较各组的生存率,多因素分析比较运用Cox比例风险模型评估。结果采用Log-rank检验提示:Ki-67>50%较Ki-67≤50%的髓母细胞瘤患者的无进展生存期(P=0.020)和总生存期(P=0.028)均缩短。GROUP 3和GROUP 4亚型的髓母细胞瘤患者较WNT和SHH亚型髓母细胞瘤患者的生存期缩短(PFS,P=0.003,OS,P=0.039)。根据Ki-67表达水平和四种分子分型对MB患者的预后不同,我们可将髓母细胞瘤患者分成低度危险组(n=12)、中度危险组(n=56)、高度危险组(n=24)(PFS,P<0.001,OS,P<0.001)。结论Ki-67表达水平及髓母细胞瘤四分型对髓母细胞瘤患者的预后有影响,可将髓母细胞瘤患者预后危险程度进行分级。Objective To investigate the effect of Ki-67 and 4 molecular subgroups on the prognosis of pa⁃tients with medulloblastoma.Methods Ninety-two cases admitted from January 2009 to January 2018 with medullo⁃blastoma confirmed by postoperative pathological were retrospectively analyzed,the Kaplan-Meier method was used to calculate the survival rates of overall survival(OS)and progression-free survival(PFS),and the survival rates of each group were compared by Log-Rank test and Cox proportional risk model assessment line multifactor analysis.Re⁃sults PFS(P=0.020)and OS(P=0.028)were shorter in patients with medulloblastoma with Ki-67>50%compared with Ki-67≤50%.Medulloblastoma patients with GROUP 3 and GROUP 4 had shorter survival compared with medul⁃loblastoma patients with WNT and SHH subgroups(PFS,P=0.003;OS,P=0.039).We stratified medulloblastoma patients into low-risk(n=12),intermediate-risk(n=56)and high-risk groups(n=24)basing on Ki-67 expression levels and different prognosis of medulloblastoma subgroups(PFS,P<0.001;OS,P<0.001).Conclusion Ki-67 ex⁃pression level and medulloblastoma subgroups have different prognosis for patients with medulloblastoma,and the prognostic risk levels of medulloblastoma patients can be stratified by Ki-67 levels and medulloblastoma subgroups.
