三维彩色血管能量成像检查联合超声弹性成像检查在宫颈癌诊断中的效能

摘　　要：目的:观察三维彩色血管能量成像检查联合超声弹性成像检查在宫颈癌诊断中的效能。方法:选取120例疑似宫颈癌患者为研究对象,均行超声弹性成像检查、三维彩色血管能量成像检查和宫颈组织病理活检。比较三维彩色血管能量成像与超声弹性成像单项检查及联合检查诊断宫颈癌的准确度,采用Kappa进行一致性检验,比较三维彩色血管能量成像单项检查、超声弹性成像单项检查和联合检查诊断宫颈癌结果与宫颈组织病理活检结果的一致性。结果:病理活检结果显示,120例高度疑似患者中,早期宫颈癌52例,晚期宫颈癌36例,非宫颈癌32例;超声弹性成像检查结果显示,早期宫颈癌46例,晚期宫颈癌37例,非宫颈癌37例;三维彩色血管能量成像检查结果显示,早期宫颈癌50例,晚期宫颈癌40例,非宫颈癌30例;超声弹性成像检查联合三维彩色血管能量成像检查结果显示,早期宫颈癌50例,晚期宫颈癌40例,非宫颈癌30例。三维彩色血管能量成像联合超声弹性成像检查诊断宫颈癌的准确度为95.00%(114/120),高于三维彩色血管能量成像单项检查的80.83%(97/120)和超声弹性成像单项检查的79.17%(95/120),差异有统计学意义(P<0.05)。Kappa一致性检验结果显示,三维彩色血管能量成像与超声弹性成像单项检查诊断宫颈癌的结果和宫颈组织病理活检结果的一致性均一般(Kappa=0.706、0.684,P<0.05);三维彩色血管能量成像联合超声弹性成像检查诊断宫颈癌与宫颈组织病理活检结果的一致性较好(Kappa=0.899,P<0.05)。结论:三维彩色血管能量成像联合超声弹性成像检查诊断宫颈癌的价值高于两者单项检查诊断。Objective:To observe efficacy of three-dimensional color power angiography combined with ultrasound elastography in diagnosis of cervical cancer.Methods:120 patients with suspected cervical cancer were selected as the research objects,and all underwent ultrasound elastography,three-dimensional color power angiography and cervical histopathological biopsy.The accuracies of single three-dimensional color power angiography,single ultrasound elastography and combined examination in the diagnosis of cervical cancer were compared.Kappa consistency check was performed to compare the consistencies of single three-dimensional color power angiography,single ultrasound elastography,combined examination and cervical histopathological biopsy.Results:The pathological biopsy results showed that among the 120 highly suspected patients,52 were diagnosed as early cervical cancer,36 were advanced cervical cancer,and 32 were non-cervical cancer.The results of ultrasound elastography showed that there were 46 cases of early cervical cancer,37 cases of advanced cervical cancer,and 37 cases of non-cervical cancer.The results of three-dimensional color power angiography showed that there were 50 cases of early cervical cancer,40 cases of advanced cervical cancer,and 30 cases of non-cervical cancer.Ultrasound elastography combined with three-dimensional color angiography showed that there were 50 cases of early cervical cancer,40 cases of advanced cervical cancer,and 30 cases of non-cervical cancer.The accuracy of three-dimensional color power angiography combined with ultrasound elastography in the diagnosis of cervical cancer was 95.00%(114/120),which was higher than 80.83%(97/120)of single three-dimensional color power angiography and 79.17%(95/120),of single ultrasound elastography,and the differences were statistically significant(P<0.05).The Kappa consistency test showed that the consistencies of the results of single three-dimensional color power angiography and single ultrasound elastography in the diagnosis of cervical cancer an
