摘　　要：目的:探讨CT纹理分析在壶腹周围癌(periampullary carcinoma,PAC)术前评估中的价值。方法:回顾性分析110例PAC患者的术前CT图像。在连续层面上沿病灶整体外缘绘制感兴趣区域(ROI),最后对每一层ROI进行合并,得到三维感兴趣容积(3D VOI)。通过独立样本t检验或Mann-Whitney U检验评估PAC患者不同组织学分型CT纹理参数的差异性。采用受试者工作特征(ROC)曲线评估CT纹理参数在PAC术前评估中的诊断效能。结果:动脉期CT纹理参数10 Percentile、90 Percentile、Energy、Mean、Median、Root Mean Squared、Total Energy在肠型PAC和胰胆管型PAC间的差异有统计学意义(均P<0.05),曲线下面积(AUC)在0.669~0.767之间。结论:动脉期CT纹理参数可为PAC术前评估提供参考。Objective:To explore the value of CT gray histogram texture analysis in the preoperative evaluation of periampullary carcinoma(PAC).Methods:Preoperative CT images of 110 patients with PAC were reviewed retrospectively.A region of interest(ROI)along the entire outer edge of the lesion on a continuous level was drawn,and finally each layer of ROI merged to get the 3D volume-of-interest(3D VOI).The Student's t test or the Mann-Whitney U test was used to evaluate the differences in histogram characteristic parameters of different histological types in patients with PAC.ROC curve was used to assess the diagnostic efficacy of CT texture parameters in the preoperative assessment of PAC.Results:In different histological classification,the parameters 10 Percentile,90 Percentile,Energy,Mean,Median,Root Mean Squared and Total Energy of the arterial phase were significantly different between the intestinal-type and pancreaticobiliary-type PAC(all P<0.05),the AUC values of these indicators ranged from 0.669 to 0.767(P<0.05).Conclusion:The CT texture parameters during the arterial phase can be useful for the preoperative evaluation of PAC.
