加味逍遥散治疗肝郁脾虚型慢性萎缩性胃炎临床疗效观察

摘　　要：目的观察加味逍遥散治疗肝郁脾虚型慢性萎缩性胃炎临床疗效。方法选取医院2018年1月—2020年1月收治的肝郁脾虚型慢性萎缩性胃炎患者96例,采用随机数字法分为两组。对照组48例给予常规对症治疗,观察组48例在对照组基础上给予加味逍遥散治疗。比较两组疗效和幽门螺杆菌(HP)转阴率,检测两组胃黏膜三叶因子2(TFF2)、核因子-κB(NF-κB)的表达及炎症炎症因子的水平。结果观察组HP转阴率和总有效率分别为91.67%(44/48)和87.50%(42/48),高于对照组的75.00%(36/48)和70.83%(P<0.05)。两组治疗前胃黏膜三叶因子2(TFF2)、核因子-κB(NF-κB)的表达水平组间比较,差异无统计学意义(P>0.05)。观察组治疗后胃黏膜TFF2、NF-κB的表达水平低于对照组(P<0.05)。两组治疗前血清炎症因子[白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、白细胞介素-11(IL-11)、白细胞介素-23(IL-23)、白细胞介素-32(IL-32)]比较,差异无统计学意义(P>0.05)。观察组治疗后炎症因子低于对照组(P<0.05)。结论加味逍遥散治疗肝郁脾虚型慢性萎缩性胃炎可减轻炎症反应,降低胃黏膜TFF2、NF-κB的表达,提高疗效,促进HP转阴。Objective To observe the clinical effect of Jiawei Xiaoyao Powder(加味逍遥散)in the treatment of chronic atrophic gastritis with liver depression and spleen deficiency.Methods From January 2018 to January 2020,96 patients with chronic atrophic gastritis with liver depression and spleen deficiency were randomly divided into two groups.In the control group,48 cases were treated with routine symptomatic treatment,and 48 cases in the observation group were treated with Jiawei Xiaoyao Powder on the basis of the control group.We compared the efficacy and Helicobacter pylori(HP)conversion rate,the expression of trifoliate factor 2(TFF2),nuclear factor-κB(NF-κB)and the level of inflammatory inflammatory factors in gastric mucosa two groups.Results The HP conversion rate and total effective rate in the observation group were 91.67%(44/48)and 87.50%(42/48),respectively,higher than those in the control group[75.00%(36/48)and 70.83%],P<0.05).There was no significant difference between the two groups in the expression levels of TFF2 and NF-κB(P>0.05).The expression levels of gastric mucosal TFF2 and NF-κB in the observation group after treatment were lower than those in the control group(P<0.05).There was no significant difference in serum inflammatory factors[interleukin-1β(IL-1β),interleukin-2(IL-2),interleukin-11(IL-11),interleukin-23(IL-23),interleukin-32(IL-32)]before treatment(P>0.05).After treatment,the inflammatory factors in the observation group were lower than those in the control group(P<0.05).Conclusion Jiawei Xiaoyao Powder can reduce inflammatory reaction,decrease the expressions of TFF2 and NF-κB in gastric mucosa,improve the curative effect and promote the negative turn of HP in chronic atrophic gastritis with liver depression and spleen deficiency.
