低分子肝素联合中药对于高脂血症型胰腺炎的疗效观察

摘　　要：目的:观察低分子肝素联合加味承气汤治疗高脂血症型胰腺炎的效果。方法:选取2020年2月-2022年2月景德镇市第一人民医院消化内科高脂血症型急性胰腺炎患者72例,依据治疗方法将其分为研究组与对照组,各36例。对照组给予小剂量低分子肝素联合加味承气汤治疗,研究组给予大剂量低分子肝素联合加味承气汤治疗。比较两组实验室指标、肾功能、急性生理与慢性健康(acute physiology and chronic health evaluation,APACHEⅡ)评分、临床疗效、临床症状缓解的时间、恢复低脂饮食时间、并发症发生情况。结果:治疗后,研究组血清甘油三酯(triacylglycerol,TG)、C反应蛋白(C reactive protein,CRP)、白细胞计数(white blood cell count,WBC)、降钙素原(procalcitonin,PCT)水平均低于对照组(P<0.05)。治疗后,研究组血肌酐(serum creatinine,Scr)、尿素氮(urea nitrogen,BUN)水平和APACHEⅡ评分均低于对照组(P<0.05)。研究组治疗总有效率为97.22%高于对照组的77.78%(P<0.05)。研究组临床症状缓解时间和恢复低脂饮食时间均短于对照组(P<0.05)。研究组并发症发生率为5.56%低于对照组的25.00%(P<0.05)。结论:与小剂量低分子肝素联合加味承气汤治疗相比,大剂量低分子肝素联合中药治疗高脂血症型胰腺炎的效果更加显著。Objective:To observe the efficacy of Low Molecular Weight Heparin combined with modified Chengqi Decoction in the treatment of hyperlipidemia pancreatitis.Method:A total of 72 patients with hyperlipidemia acute pancreatitis in Department of Gastroenterology in Jingdezhen First People’s Hospital from February 2020 to February 2022 were selected,and they were divided into study group and control group according to treatment methods,with 36 cases in each group.The control group was given low dose of Low Molecular Weight Heparin combined with modified Chengqi Decoction,and the study group was given high dose of Low Molecular Weight Heparin combined with modified Chengqi Decoction.The laboratory indicators,renal function,acute physiology and chronic health evaluation(APACHEⅡ)score,clinical efficacy,the time of clinical symptom remission,the time to resume low-fat diet and the incidence of complications were compared between the two groups.Result:After treatment,the levels of serum triacylglycerol(TG),C reactive protein(CRP),white blood cell count(WBC),procalcitonin(PCT)in the study group were lower than those in the control group(P<0.05).After treatment,the levels of serum creatinine(Scr),urea nitrogen(BUN)and APACHEⅡscore in the study group were lower than those in the control group(P<0.05).The total effective rate in the study group was 97.22%higher than 77.78%in the control group(P<0.05).The time of clinical symptoms remission and the recovery time of low-fat diet in the study group were shorter than those in the control group(P<0.05).The complication rate of study group was 5.56%,lower than 25.00%of control group(P<0.05).Conclusion:Compared with low dose of Low Molecular Weight Heparin combined with modified Chengqi Decoction,high dose of Low Molecular Weight Heparin combined with modified Chengqi Decoction has a more significant effect on the treatment of hyperlipidemia pancreatitis.
