吲哚菁绿染色联合全氟丙烷填充治疗高度近视黄斑裂孔视网膜脱离的效果分析

摘　　要：目的探究高度近视黄斑裂孔视网膜脱离应用吲哚菁绿染色联合全氟丙烷(C_(3)F_(8))填充治疗的临床效果。方法64例(64眼)高度近视黄斑裂孔视网膜脱离患者,随机分为A组和B组,各32例(32眼)。A组予以术后玻璃体腔填充全氟丙烷治疗,B组予以吲哚菁绿染色联合全氟丙烷填充治疗。比较两组患者视网膜复位情况、治疗安全性及术后不同时间裸眼对数视力、眼压。结果B组患者视网膜复位成功率90.63%高于A组的68.75%,差异具有统计学意义(P<0.05)。B组患者术后7、30、90及120 d的裸眼视力高于A组,差异有统计学意义(P<0.05)。术后7、30 d,B组患者眼压分别为(13.30±1.44)、(13.22±1.34)mm Hg(1 mm Hg=0.133 kPa),均低于A组的(17.32±1.39)、(16.12±2.34)mm Hg,差异有统计学意义(P<0.05);术后90、120 d时,A组与B组患者的眼压比较,差异无统计学意义(P>0.05)。两组患者并发症发生率比较,差异无统计学意义(P>0.05)。结论对高度近视黄斑裂孔视网膜脱离患者应用吲哚菁绿染色+全氟丙烷填充治疗效果确切,能够促进患者视力改善,提升其视网膜复位效果,且术后并发症少,建议推广。Objective To investigate the clinical effect of indocyanine green staining combined with perfluoropropane(C_(3)F_(8))filling in the treatment of patients with retinal detachment caused by macular hole in high myopia.Methods A total of 64 patients(64 eyes)with retinal detachment caused by macular hole in high myopia were randomly divided into group A and group B,each with 32 cases(32 eyes).Group A received postoperative vitreous cavity filling with perfluoropropane,and group B received indocyanine green staining combined with perfluoropropane filling.The retinal reattachment,treatment safety,uncorrected logarithmic visual acuity and intraocular pressure at different time points after surgery were compared between the two groups.Results The success rate of retinal reattachment 90.63%in group B was higher than 68.75%in group A,and the difference was statistically significant(P<0.05).The uncorrected visual acuity at 7,30,90 and 120 d after surgery in group B was higher than those in group A,and the difference was statistically significant(P<0.05).At 7 and 30 d after surgery,the intraocular pressure of group B were(13.30±1.44)and(13.22±1.34)mm Hg(1 mm Hg=0.133 kPa),which were lower than(17.32±1.39)and(16.12±2.34)mm Hg of group A,and the difference was statistically significant(P<0.05).At 90 and 120 d after surgery,there was no statistically significant difference in intraocular pressure between group A and group B(P>0.05).There was no statistically significant difference in complication rate between the two groups(P>0.05).Conclusion Indocyanine green staining in combination with perfluoropropane filling is effective in the treatment of patients with retinal detachment caused by macular hole in high myopia,and can promote the improvement of visual acuity and enhance the effect of retinal reattachment with few postoperative complications,which is recommended to be promoted.
