摘 要:目的探究PAX-1基因甲基化在人乳头瘤病毒(HPV)阳性宫颈癌患者中的诊断价值。方法120例宫颈癌患者,依据HPV检查结果分为阳性组(HPV阳性)与阴性组(HPV阴性),各60例。另选择同期HPV阳性而宫颈正常的患者60例作为对照组。三组患者均进行甲基化特异性聚合酶链反应法(MSP)检测。观察比较三组PAX-1基因甲基化的表达阳性率,分析PAX-1基因甲基化检测结果与宫颈癌病理特征的关系。结果阳性组中PAX-1基因甲基化的表达阳性率为96.67%(58/60),阴性组中PAX-1基因甲基化的表达阳性率为83.33%(50/60),对照组中PAX-1基因甲基化的表达阳性率为11.67%(7/60)。阳性组中PAX-1基因甲基化的表达阳性率高于阴性组与对照组,且阴性组高于对照组,差异有统计学意义(P<0.05)。PAX-1基因甲基化检测结果与宫颈癌患者年龄、肿瘤大小、组织分化程度、FIGO分期无关(P>0.05)。PAX-1基因甲基化检测结果与宫颈癌患者淋巴结转移有关(P<0.05)。结论PAX-1基因甲基化在宫颈癌组织中呈高表达状态,而在HPV阳性宫颈癌中表达呈更高状态,通过对PAX-1基因甲基化水平的检测可以指导临床诊断、治疗宫颈癌。Objective To investigate the value of PAX-1 methylation in the diagnosis of human papillomavirus(HPV)-positive patients.Methods A total of 120 patients with cervical cancer were divided into positive group(HPV positive)and negative group(HPV negative)according to the HPV test results,with 60 cases in each group.Another 60 patients with normal cervix and positive HPV during the same period were selected as the control group.All three groups were detected by methylation-specific polymerase chain reaction(MSP).The positive rate of PAX-1 gene methylation of the three groups was observed and compared,and the correlation between the methylation of PAX-1 gene and the pathological characteristics of cervical cancer was analyzed.Results The positive rate of PAX-1 gene methylation in the positive group was 96.67%(58/60),the positive rate of PAX-1 gene methylation expression in the negative group was 83.33%(50/60),and the positive rate of PAX-1 gene methylation in the control group was 11.67%(7/60).The positive rate of PAX-1 gene methylation expression in the positive group was higher than that in the negative group and the control group,and the negative group was higher than the control group.All the differences were statistically significant(P<0.05).PAX-1 gene methylation was not significantly correlated with age,tumor size,degree of tissue differentiation,and FIGO stage in cervical cancer patients(P>0.05).PAX-1 gene methylation was associated with lymph node metastasis in patients with cervical cancer(P<0.05).Conclusion PAX-1 gene methylation is highly expressed in cervical cancer tissues,and the expression is higher in HPV-positive cervical cancer.The detection of PAX-1 gene methylation can guide clinical diagnosis and treatment of cervical cancer.
