摘  要:目的探讨MRI序列定量评估离体猪肝微波消融效果的可行性,为其临床应用提供实验依据。方法采用4个新鲜离体猪肝标本进行微波消融,交替以50 W/5 min、60 W/5 min共制备24个消融灶,在消融前和消融后即刻进行MRI扫描,成像序列为T_(1)-3D WATS、T_(2)-SPIR、DWI和m-Dixon quant序列,采用随机区组设计的方差分析比较术前对照组(n=24)、50W组(n=12)及60W组(n=12)靶区脂肪分数(FF)、T_(2)^(*)值、R_(2)^(*)值和ADC值的测量差异,采用Spearman相关分析消融后靶区定量参数的变化(△)与消融功率选择的相关性。结果对照组、50 W组、60 W组靶区的总体FF、T_(2)^(*)值、R_(2)^(*)值、ADC值及消融后不同测量区域(靶区中心、靶区外带、靶区周围)的FF、T_(2)^(*)值、R_(2)^(*)值、ADC值差异比较均具有统计学意义(P<0.05);靶区及靶区周围的△FF、△T_(2)^(*)、△R_(2)^(*)、△ADC与消融功率选择均具有较好的相关性,其相关系数r分别为0.877、-0.769、0.859、-0.555,以及-0.579、0.857、-0.865、0.797,P均<0.05。结论FF、T_(2)^(*)值、R_(2)^(*)值能较为准确的定量评估离体猪肝的MWA效果,m-Dixon quant与DWI联合将有助于提高MRI定量评估消融疗效的准确性。Objective To explore the feasibility of using MRI sequences to quantitative evaluate the effect of microwave ablation(MWA)in ex vivo porcine liver,and to provide relevant reference for clinical application.Methods Four fresh ex vivo porcine livers were used for MWA,totally formed 24 ablation zones with energy at 50 W/5 min and 60W/5 min alternately.MRI imaging(T_(1)-3D WATS,T_(2)-SPIR,m-Dixon Quant and DWI)was performed before and after MWA immediately.All data were divided into 3 groups(preoperative control group,n=24;50 W group,n=12;60 W group,n=12);Analysis of variance for random block design was used to compare the differences of fat fraction(FF),T_(2)^(*)value,R_(2)^(*)value and ADC value among three groups and in the different areas of MWA;Spearman correlation was used to analyze the correlation between△parameters and microwave energy.Results All parameters(FF,T_(2)^(*),R_(2)^(*)and ADC)showed significant difference among the 3 groups and in the different areas(central zone,outer zone,peripheral zone)after MWA with all P values<0.05;The variations(△FF,△T_(2)^(*),△R_(2)^(*),△ADC)in ablation zone and peripheral zone both revealed highly correlation between quantitative parameters and generate power of MWA,the correlation coefficient r respectively was 0.877,-0.769,0.859,-0.555 in ablation zone,and respectively was-0.579,0.857,-0.865,0.797 in peripheral zone,all P values<0.05.Conclusion FF,T_(2)^(*)value and R_(2)^(*)value were confirmed to accurately evaluate the effect of MWA,combined m-Dixon quant and DWI could improve the reliability of MRI quantitative evaluation for hepatic MWA.
