二例单发原发性骨淋巴瘤的^(18)F-FDG PET/CT鉴别诊断

摘  要:原发性骨淋巴瘤(PBL)是一种罕见的结外淋巴瘤, 单发病灶的PBL更为罕见, 其影像学表现有溶骨破坏、软组织肿块、骨质密度不均等, 形态学上缺乏特异性, 仅从病变部位的CT或MRI的影像学表现很难做出准确诊断。笔者选取了2例行^(18)F-FDG PET/CT显像且经组织病理学检查证实为单发病灶的PBL患者, 收集患者的临床资料, 从其临床特征和PET/CT影像学表现等方面综合分析, 与其他骨恶性肿瘤及骨良性疾病等进行鉴别、诊断, 总结分析诊断思路, 得出相对准确的影像学诊断, 为临床提供帮助。Primary bone lymphoma is a rare type of extra-lymph node lymphoma,especially when it is a single lesion.Its imaging manifestations include osteolytic destruction,soft tissue mass,uneven bone density,it is especially difficult to make a diagnosis only from the morphological manifestations of the lesion site on CT or MRI.The author selected two cases of pathologically confirmed bone lymphomas with ^(18)F-FDG PET/CT imaging,and both of which were primary bone lymphomas with single lesion.The clinical data of the patients were collected,and their clinical features and PET/CT imaging manifestations were comprehensively analyzed to differentiate and diagnose them from other bone malignant tumors and benign bone diseases,etc.The diagnostic ideas were summarized and analyzed to arrive at a relatively accurate imaging diagnosis to help the clinic.
