玻璃酸钠联合睑板腺按摩治疗睑板腺功能障碍性干眼的效果

摘　　要：目的评价玻璃酸钠滴眼液滴眼联合睑板腺按摩治疗睑板腺功能障碍(MGD)性干眼的效果。方法前瞻性随机对照研究。以便利抽样法对三门峡市中心医院2019年5月至2020年12月收治的MGD性干眼120例开展研究,依照随机数字表法分为两组:对照组60例,行睑板腺按摩术;观察组60例,行玻璃酸钠滴眼液滴眼联合睑板腺按摩术,两组均坚持治疗8周,比较两组眼表疾病指数(OSDI)、干眼症状评估(SPEED)非接触式泪膜破裂时间(NITBUT)、角膜荧光素染色(CFS)、泪液分泌试验(SIt)评分。结果两组治疗前OSPI、SPEED评分比较,差异无统计学意义(P>0.05),治疗后观察组OSDI和SPEED评分分别为(22.79±8.55)分和(7.56±1.65)分,均低于对照组的(30.63±10.24)分和(9.57±1.86)分(均P<0.05);两组治疗前NITBUT、CFS、SIt比较,差异均无统计学意义(均P>0.05)。治疗后两组NITBUT、SIt均呈上升趋势,CFS则降低,观察组NITBUT、SIt分别为(8.12±2.05)s和(8.16±1.35)mm/5 min,均高于对照组的(6.79±1.78)s和(7.02±1.54)mm/5 min,CFS为(0.75±0.24)min,低于对照组的(1.06±0.41)min(均P<0.05);两组并发症及不良反应发生率比较,差异无统计学意义(P>0.05)。结论MGD性干眼应用玻璃酸钠滴眼液滴眼联合睑板腺按摩治疗可获确切效果,不仅症状及体征均有改善,而且并发症及不良反应较少,具有较高安全性。Objective To evaluate the efficacy of sodium hyaluronate eye drops combined with meibomian gland massage for the treatment of meibomian gland dysfunction(MGD)dry eye.Methods This was a prospective randomized controlled study.Total of 120 patients with MGD dry eye treated in Sanmenxia Central Hospital from May 2019 to Dec.2020 were studied by convenience sampling method.Based on random number table method,they were divided into two groups,with 60 cases in each group.The control group received meibomian gland massage and the observation group received sodium hyaluronate eye drops combined with meibomian gland massage.The two groups were treated for 8 weeks,and the ocular surface disease index(OSDI)and standard patient evaluation of eye dryness(SPEED)scores non-invasive breakup time(NITBUT),corneal fluorescein staining(CFS)and SchirmerⅠtest(SIt)between the two groups were compared.Results There were no significant differences in OSDI and SPEED scores between the two groups before treatment(P>0.05).After treatment,OSDI and SPEED scores in the observation group were(22.79±8.55)points and(7.56±1.65)points,respectively,lower than(30.63±10.24)points and(9.57±1.86)points in the control group(all P<0.05).Before treatment,there was no significant differences in NIBUT,CFS,SIt between the two groups(all P>0.05).After treatment,NITBUT and SIt increased in both groups,while CFS decreased.In the observation group,NITBUT and SIt were(8.12±2.05)s and(8.16±1.35)mm/5 min,respectively,those were all higher than the(6.79±1.78)s and(7.02±1.54)mm/5 min of the control group.CFS was(0.75±0.24)min,which was lower than the control group(1.06±0.41)points(P<0.05).There was no significant difference in the incidence of complications and adverse reactions between the two groups(P>0.05).Conclusion The treatment of MGD dry eye with sodium hyaluronate eye drops combined with meibomian gland massage can achieve a definite curative effect,not only the symptoms and signs are improved,but also fewer complications and adverse reactions,with
