术前散光轴向对散光矫正型人工晶状体植入效果的影响

摘　　要：目的观察术前角膜散光的轴向对于散光矫正型(toric)人工晶状体植入效果的影响。方法回顾性病例对照研究。分析郑州爱尔眼科医院2017年6月至2020年12月术前有角膜散光的白内障患者超声乳化手术128例(180眼)的临床资料,患者分为两组,每组90眼。Toric组,植入Acrysof IQ toric人工晶状体;IQ组,植入Acrysof IQ人工晶状体。亚组系按照术前角膜散光轴向分为顺规散光组、逆规散光组及斜轴散光组。术后3个月比较两组视力及残余散光度等。结果术后裸眼远视力Toric组优于IQ组(P<0.05),残余散光度Toric组低于IQ组(P<0.05)。逆规散光患者及斜轴散光患者术后裸眼远视力Toric组优于IQ组(P<0.05),而顺规散光患者的两组差异无统计学意义(P>0.05)。结论对于上方角膜切口,toric人工晶状体相比IQ人工晶状体矫正逆规散光和斜轴散光的效果比顺规散光好。Objective To observe the influence of the axis of corneal astigmatism before operation on the outcomes of Toric intraocular lens implantation.Methods This was a retrospective case-control study.The clinical data of 180 eyes of 128 cataract patients with corneal astigmatism who underwent phacoemulsification from Jun.2017 to Dec.2020 in Zhengzhou Aier Ophthalmic Hospital were analyzed.The patients were divided into two groups.In the toric group,90 eyes implanted AcrySof IQ toric intraocular lens.In the IQ group,90 eyes implanted AcrySof IQ aspheric intraocular lens.All patients were divided into with the rule group,against the rule group and oblique astigmatism group based on the preoperative astigmatism axial direction.The visual acuity and residual astigmatism between the two groups were compared at 3 months after operation.Results The uncorrected distant visual acuity(UDVA)in the Toric group was better than that in the IQ group(P<0.05),and residual astigmatism in the Toric group was significantly lower than that in IQ group(P<0.05).For patients with against the rule(ATR)astigmatism and oblique astigmatism,the uncorrected distant visual acuity in the toric group was better than that in the IQ group,while in patients with the rule(WTR)astigmatism,there was no significant difference between the two groups(P>0.05).Conclusion For the superior corneal incision,the efficacy of Toric intraocular lens in the correction of ATR astigmatism and oblique astigmatism is more significant than that in the correction of WTR compared to the IQ group.
