摘　　要：目的随访鼻咽癌放患者化疗出院3年后的康复及生活质量现状,分析生活质量的影响因素。方法采用自行设计的一般资料及康复情况调查表和中文版EQ-5D量表对118例鼻咽癌放化疗出院后患者进行问卷调查,分析鼻咽癌患者放化疗出院后康复及生活质量现状及影响因素。结果118例患者中46例(39%)的患者已就业,25例(21.20%)患者颈部僵硬。所有患者的EQ-5D效用值为0.877±0.048,常有疼痛/不舒服、焦虑/抑郁方面的问题。多元逐步回归分析显示鼻咽冲洗、张口度、颈部僵硬、是否就业是EQ-5D效用值的影响因素(P<0.05)。结论临床医护人员应关注鼻咽癌患者出院后及中长期生存的康复情况和生活质量,构建针对鼻咽癌患者出院后症状集群特点和随着时间变化规律的延续护理方案,提高鼻咽癌幸存患者生活质量。Objective To investigate the status of rehabilitation and quality of life in patients with nasopharyngeal carcinoma(NPC)after 3 years of radiotherapy and chemotherapy discharge,and to analyze the influencing factors of quality of life.Methods The self-designed general information and rehabilitation status questionnaire and Chinese version EQ-5D scale were used to investigate 118 patients with NPC discharged from hospital after radiotherapy and chemotherapy.The status and influencing factors of rehabilitation and quality of life of the patients after radiotherapy and chemotherapy were analyzed.Results Of the 118 patients,46(39%)were employed,and 25(21.20%)had neck stiffness.The EQ-5D utility value was 0.877±0.048,and often had pain/discomfort,and anxiety/depression problems.Multiple stepwise regression analysis showed that nasopharyngeal irrigation,mouth opening degree,neck stiffness,age and employment were the influencing factors of EQ-5D utility value(P<0.05).Conclusion Clinical medical staff should pay attention to the rehabilitation and quality of life of patients with NPC after discharge and the long-term survival.A continuous nursing plan according to the characteristics of symptom cluster should be conducted and the changed over time,so as to improve the quality of life of patients with nasopharyngeal cancer.
