52例儿童肾母细胞瘤临床诊治分析

摘　　要：目的探讨儿童肾母细胞瘤的临床病理特征、诊治方式与预后。方法回顾性分析2013年1月至2018年12月郑州大学第一附属医院小儿外科收治的肾母细胞肿瘤患儿的临床病理资料、诊治方法以及预后状况。结果符合条件的52例肾母细胞肿瘤患儿纳入研究,其中男28例、女24例。Ⅰ~Ⅱ期患儿的完全缓解率明显大于Ⅲ~Ⅳ期患儿(100.00%、42.10%;χ^(2)=14.046,P<0.001),Ⅲ期患儿的完全缓解率显著高于Ⅳ期患儿(53.85%、23.07%,χ^(2)=15.707,P<0.001)。Kaplan-Meier法绘制患儿生存曲线,log rank检验结果显示Ⅲ期和Ⅳ期患儿预后相近(χ^(2)=2.491,P=0.329)。结论儿童肾母细胞肿瘤的总生存率比较高,但Ⅳ期患儿的总体预后不好,远处转移则以肺转移最多见。Objective To study the clinicopathological features,diagnosis and treatment methods and prognosis of Wilms tumor in children.Methods The clinicopathological data,treatment methods and prognosis of children with Wilms tumor admitted to the Department of Pediatric Surgery of the First Affiliated Hospital of Zhengzhou University from January 2013 to December 2018 were retrospectively analyzed.Results Fifty-two children with complete medical records and long-term follow-up were included in the study,including 28 males and 24 females.The complete remission rate of stageⅠandⅡchildren was significantly higher than that of stageⅢandⅣchildren(100.00%and 42.10%,χ^(2)=14.046,P<0.001);the complete remission rate of stageⅢchildren was significantly higher than that of stageⅣchildren(53.85%and 23.07%,χ^(2)=15.707,P<0.001).The Kaplan-Meier method was used to describe the survival curve of children,and the log rank test showed that the prognosis of patients with stageⅢandⅣwas not statistically significant(χ^(2)=2.491,P=0.329).Conclusion The overall survival rate of children with Wilms tumor is relatively high,but the overall prognosis of stage IV patients is not good,and lung metastasis is the most common distant metastasis.
