云南省某三级甲等医院2015年-2019年罕见病住院病例分析

摘　　要：目的 分析云南省某三甲医院收治的住院患者有罕见病的病例特点,为云南省制定罕见病防治策略提供参考依据。方法 依据罕见病目录中ICD-10编码在患者住院病案首页中对主要诊断和次要诊断进行筛选,收集该医院2015年1月1日-2019年11月30日罕见病患者共1757例,剔除重复病例后对1564例住院患者数据进行流行病学描述和分析。结果 1564例病例占全部出院病例的0.24%,涉及20个病种,其中病例数居前五位的罕见病占病例数的85.18%,前三位依次是自身免疫性垂体炎、全身型重症肌无力、肌萎缩侧索硬化,分别占48.21%、16.18%、11.96%。男性772例(49.36%),女性792例(50.64%),总体性别分布无统计学差异。从年龄构成来看,年龄最大92岁,最小为刚出生的先天性高胰岛素性低血糖症患儿,45-54岁患者占比(23.53%)最高,共368例。结论 该医院就诊的患者中有罕见病诊断的为1564例,推测云南省有较大的罕见病患病人群,在系统分布方面与其它地区的罕见病研究差异较大,且婴幼儿病例占比不高,应进一步提高罕见病诊断能力,早发现早诊断。Objective To analyzethe characteristics of hospitalized patients with a triprase hospital in Yunnan Province have a rare diagnosis of cases,providing a reference basis for developing rare disease prevention strategies in Yunnan Province.Methods According to the rare disease catalog,ICD-10 encoding is screened for major diagnosis and secondary diagnosis in the homepage of hospitalized patients.1757 patients with rare diseases from January 1,2015 to November 30,2019,excluding During the repetition,epidemiological descriptions and analysis of 1564 patients patients were repeated.Results 1564 cases accounting for 0.24% of all discharge cases,involving 20 diseases,wherein the number of cases rarely patients account for 85.18.%,The top three in turn is autoimmune pititis,systemic intensive muscle weakness,muscle atrophy lateral,with 48.21%,16.18%,11.96%respectively.The male is 772(49.80%),792 females(50.64%),and the overall gender distribution has no statistical differences.From the age of age,the age of 92 years old,the smallest is the newly born hyperloryin hypoglycemia,and the 45-54-year-old patient accounts for the highest(23.53%),368 patients.Conclusion There is 1564 cases of rare disease diagnosis in patients in the hospital.It is speculated that there is a large number of rare diseases in Yunnan Province,and the rare diseases in system distribution and rare diseases in other regions have different differences,and infants and young children account for proportion.Not high,should further improve the diagnostics of rare diseases,and early diagnosis.
