血清中miR-10b、LMP1和Twist1联合检测在鼻咽癌早期诊断及复发诊断中的应用

摘　　要：目的探讨血清中微小核糖核酸-10 b(miR-10b)、潜伏膜蛋白1(LMP1)和人Twist相关蛋白1(Twist1)联合检测在鼻咽癌(NPC)早期诊断和复发诊断中的应用价值.方法选取113例NPC患者(记为NPC组)和45例鼻咽炎患者(记为鼻咽炎组)为研究对象,依据NPC组患者随访结果分为复发组和无复发组,比较各组血清miR-10b、LMP1和Twist1相对表达量,应用ROC曲线评估联合检测血清miR-10b、LMP1和Twist1在NPC早期诊断及复发诊断中的应用价值.结果NPC组血清miR-10b、LMP1和Twist1相对表达量明显高于鼻咽炎组(P<0.05).Twist1阴性表达组、Twist1阴性而LMP1阳性表达组、Twist1、LMP1同时阳性表达组miR-10b表达水平依次明显增高,组间两两比较差异显著(P<0.05).血清miR-10b、LMP1和Twist1三项联合(串联)检测预测NPC的价值较高(特异度、ROC曲线下面积分别为86.5%、0.860),较单独检测高,而血清miR-10b预测NPC的ROC曲线下面积、灵敏度较LMP1、Twist1高,临界值为2.31、1.92、1.87.校正混杂因素后提示血清miR-10b(OR=1.523,95%CI:1.216~1.909)、LMP1(OR=1.523,95%CI:1.029~2.142)和Twist1相对表达量(OR=1.523,95%CI:1.052~2.189)是NPC患者复发的影响因素(P<0.05).血清miR-10b、LMP1和Twist1表达联合(串联)检测预测NPC患者复发的特异度(89.9%)、ROC曲线下面积(0.892)高于单一检测.结论血清中miR-10b、LMP1和Twist1表达与NPC发生及复发密切相关,三者可作为NPC早期诊治的有效标志物.Objective To investigate the application value of joint detection of serum micro ribonucleic acid-10b(miR-10b),EB virus-encoded latent membrane protein 1(LMP1)and Twist1 in early diagnosis and recurrence diagnosis of nasopharyngeal carcinoma(NPC).Method 113 patients with NPC(NPC group)and 45 patients with nasopharyngitis(nasopharyngitis group)were selected as the research subjects.According to the follow-up results,patients in the NPC group were divided into recurrence group and non-recurrence group.The relative expression levels of serum miR-10b,LMP1 and Twist1 in the two groups were compared.The ROC curve was used to evaluate the application value of joint detection of serum miR-10b,LMP1 and Twist1 in early diagnosis and recurrence diagnosis of NPC.Results The relative expression levels of serum miR-10b,LMP1 and Twist1 in the NPC group were significantly higher than those in the nasopharyngitis group(P<0.05).The expression level of miR-10b in Twist1 negative expression group,Twist1 negative and LMP1 positive expression group,Twist1 and LMP1 simultaneous positive expression group increased significantly in turn.There was significant difference between the two groups(P<0.05).The specificity and area under the ROC curve of joint detection of serum miR-10b,LMP1 and Twist1 in predicting NPC were 86.5%and 0.860,which were higher than those of single detection.The area under the ROC curve and the sensitivity of serum miR-10b in predicting NPC were higher than those of LMP1 and Twist1.The cut-off values were 2.31,1.92 and 1.87.After adjusting for confounding factors,it was found that the relative expression levels of serum miR-10b(OR=1.523,95%CI:1.216~1.909),LMP1(OR=1.523,95%CI:1.029~2.142)and Twist1(OR=1.523,95%CI:1.052~2.189)were influencing factors of recurrence in patients with NPC(P<0.05).The specificity and area under the ROC curve of joint detection of serum miR-10b,LMP1 and Twist1 in predicting recurrence of NPC(89.9%,0.892)were higher than those of single detection.Conclusion The expression of serum miR-10b,LM
