金属丝-树脂夹板固定成人外伤性前牙的效果及对牙周微生态环境和美观性的影响

摘　　要：目的:探讨金属丝-树脂夹板固定成人外伤性前牙的效果及对牙周微生态环境、美观性的影响。方法:按照随机数字表法将2018年1月-2019年12月笔者医院收治的80例(80颗患牙)前牙外伤患者分为对照组与观察组,每组40例。对照组患者采用牙弓夹板固定,观察组患者采用金属丝-树脂夹板固定。4周后拆除夹板,比较两组临床疗效,牙周微生态环境指标[菌斑指数(PLI)、龈沟出血指数(SBI)、牙龈指数(GI)和牙周袋深度(PD)]、红色美学评分(PES)及患者满意度评分。结果:治疗结束时,观察组与对照组的治疗有效率分别为95.00%、80.00%,观察组明显高于对照组,差异有统计学意义(P<0.05);观察组的PLI、SBI、GI、PD及龈沟液IL-1β、IL-4、hs-CRP水平明显低于对照组,差异有统计学意义(P<0.05);观察组患者对整体美观满意度评分明显高于对照组,差异有统计学意义(P<0.05)。治疗结束时及治疗后3、6个月,观察组的PES评分均明显高于对照组,差异有统计学意义(P<0.05)。结论:金属丝-树脂夹板在成人外伤性前牙固定中的疗效显著,对牙周微生态环境影响较小,美学效果好,患者满意度高。Objective To investigate the efficacy of metal wire-resin splint in the fixation of traumatic anterior tooth in adults and its effect on peridental microecological environment and aesthetics.Methods According to the random number table method,80 patients(80 affected teeth)with anterior tooth trauma who were divided into control group and observation group in our hospital from January 2018 to December 2019,with 40 cases in each group.The patients were fixed with dental arch splint in control group,the patients were fixed with metal wire-resin splint in observation group,and the splin was removed after 4 weeks.The clinical efficacy,peridental microecological environment indexes[plaque index(PLI),gingival sulcus bleeding index(SBI),gingival index(GI),periodontal pocket depth(PD)],pink esthetic score(PES)and patient’s satisfaction score was compared between two groups.Results At the end of treatment,the effective rate of observation group and control group was 95.00%and 80.00%respectively,the observation group was significantly higher than control group(P<0.05).The PLI,SBI,GI,PD and levels of gingival crevicular fluid IL-1β,IL-4,hs-CRP in observation group was significantly lower than that in control group(P<0.05).The patient’s satisfaction score of overall aesthetics and masticatory function in observation group was significantly higher than that in control group(P<0.05).At the end of treatment and 3,6 months after treatment,the PES score in observation group was significantly higher than that in control group(P<0.05).Conclusion The metal wire-resin splint had obvious curative effect,good aesthetic effect and high patient’s satisfaction in the fixation of traumatic anterior tooth in adults,and it had little effect on the periodontal microecological environment.
