方丝片段弓技术与牙弓夹板固定术在外伤脱位牙再植术中的应用对比研究

摘　　要：目的:比较外伤脱位牙再植术中方丝片段弓技术与牙弓夹板固定术的应用效果。方法:选择2015年1月-2020年12月笔者医院行外伤脱位牙再植术的137例患者(共脱位137颗牙齿)为研究对象,根据治疗方法不同,分为对照组(70例,牙弓夹板固定术)和观察组(67例,方丝片段弓技术)。分析两组患者的临床疗效、固定质量、美观度、舒适度评分情况及术后6个月的牙齿松动程度。结果:治疗后,观察组总有效率(98.51%)明显高于对照组(90.00%),差异有统计学意义(P<0.05);观察组术后6个月主观满意度评分项目(美观性、舒适度、固定质量)均显著高于对照组,差异有统计学意义(P<0.05);观察组术后Ⅰ度松动率(62.69%)高于对照组(45.71%),差异有统计学意义(P<0.05);观察组Ⅱ度松动率(34.33%)与对照组(41.43%)比较,差异无统计学意义(P>0.05);观察组Ⅲ度松动率(2.98%)明显低于对照组(12.86%),观察组牙齿脱位复发率(2.99%)显著低于对照组(12.86%),差异均有统计学意义(P<0.05)。结论:方丝片段弓技术临床总有效率显著,且美观性、舒适度、固定质量都优于牙弓夹板固定术,临床应用价值高。Objective To compare the application effect of the edgewise arch wire technology and arch barsplint fixation in replantation of traumatic dislocation teeth.Methods The 137 patients(137traumatic dislocation teeth in total)who were treated with replantation of traumatic dislocation teeth in this hospital from January 2015 to December 2020 were divided into a control group(70 cases,arch barsplint fixation)and an observation group(67 cases,edgewise arch wire technology)according to different treatment methods.The clinical efficacy,fixation quality,aesthetics,and comfort scores and the degree of tooth looseness of patients 6 months after surgery between the two groups were analyzed.Results The total effective rate of the observation group after treatment was 66 cases(98.51%),which was significantly higher than the 63 cases(90.00%)of the control group(P<0.05).The subjective satisfaction scores(beauty,comfort,and fixation quality)of the observation group 6 months after operation were significantly higher than those of the control group(P<0.05).The postoperative first-degree loosening rate of the observation group was 62.69%,which was higher than the 45.71%of the control group(P<0.05).There was no significant difference in the postoperative second-degree loosening rate between the observation group 34.33%and the control group 41.43%(P>0.05).The third degree loosening rate of the observation group was 2.98%,which was significantly lower than 12.86%of the control group(P<0.05).The recurrence rate of tooth dislocation in the observation group was 2.99%,which was significantly lower than 12.86%in the control group(P<0.05).Conclusion The edgewise arch wire technology and arch barsplint fixation has a significant clinical efficiency,and its aesthetics,comfort,and fixation quality are superior to those of dental arch splint fixation,and the clinical application value is high.
