摘 要:脑小血管病(cerebral small vessel disease,CSVD)是造成老年人认知功能受损和日常生活能力下降的主要原因之一,对其准确评估日益受到关注。近年来,CSVD的影像学评估正逐渐从传统视觉评估转向更精准的半自动或全自动测量。本文着重对CSVD的六种影像学标志物,即白质高信号(white matter hyperintensities,WMH)、血管周围间隙(perivascular spaces,PVS)、近期皮质下小梗死(recent small subcortical infarcts,RSSI)、脑微出血(cerebral microbleeds,CMB)、腔隙及脑萎缩的影像学自动测量进展进行综述。自动测量相比传统评估方法不仅更便捷和准确,还非常利于纵向对比和与神经网络研究相结合。未来CSVD的影像学检测面临的挑战将不仅是自动切割方式的改进,更需要解决多种成像结果综合运用、多种分析方法有效整合的难题。Cerebral small vessel disease(CSVD)is one of the leading causes of cognition decline and quality of life erosion in the elderly.Therefore,the precise assessment of CSVD has gained more and more attentions.In recent years,image assessment of CSVD has gradually moved forward from traditional visual-evaluations to the more subtle semiautomated or automated approaches.In this review,we focused on the recent advances in the automated image assessment of the six radiological biomarkers of CSVD,i.e.,white matter hyperintensities(WMH),perivascular spaces(PVS),recent small subcortical infarcts(RSSI),cerebral microbleeds(CMB),launce and brain atrophy.Compared with traditional visual evaluation,automated image assessment is not only more convenient and accurate,but also very useful for longitudinal comparison and the combination with neural network research.In the future,the challenge of image assessment of CSVD will be faced in not only in improving approaches of automatic segmentation,but also in solving the problem of integration of different sequences of images and multiple analysis methods.
