摘　　要：目的 评估国产来曲唑联合卵巢功能抑制治疗高复发风险绝经前乳腺癌的疗效。方法 本研究选择2014年12月1日~2016年12月30日贵州省人民医院诊治的232例高复发风险激素受体阳性的绝经前乳腺癌患者进行前瞻性对照研究。根据入组先后顺序,分为国产来曲唑+卵巢功能抑制(OFS)组(n=124)和他莫昔芬(TAM)+OFS组(n=108),均联合应用戈舍瑞林行卵巢功能抑制,评价两组患者的临床疗效、毒副反应及生活质量。结果 国产来曲唑+OFS组患者的中位BCFI为60(55.3~64.7)个月、中位DFS为45(39.7~50.3)个月;TAM+OFS组患者的中位BCFI为38(35.9~40.1)个月,中位DFS为34(30.2~37.8)个月。两组患者BCFI、DFS差异均有统计学意义(P <0.05),不良反应及生活质量差异无统计学意义(P> 0.05)。结论 国产来曲唑联合OFS治疗高复发风险的绝经前乳腺癌患者,较TAM能够延缓其复发、转移风险,对患者生活质量无明显影响。Objective To evaluate the efficacy of domestic letrozole combined with ovarian function suppression in the treatment of premenopausal breast cancer with high risk of recurrence.Methods This study selected 232 premenopausal breast cancer patients with high risk of recurrence and hormone receptor positive who were diagnosed and treated in Guizhou Provincial People’s Hospital from December 1,2014 to December 30,2016 for a prospective controlled study.According to the order of entry,they were divided into domestic letrozole + ovarian function suppression(OFS) group(n =124) and tamoxifen(TAM) + OFS group(n = 108),both of which were combined with goserelin inhibition of ovarian function,to evaluate the clinical efficacy,side effects and quality of life of the two groups of patients.Results In the domestic letrozole + OFS group,the median BCFI was 60(range:55.3 ~ 64.7) months and the median disease-free survival(DFS) was 45(range:39.7 ~ 50.3) months.In the TAM + OFS group,the median BCFI was 38(range:35.9 ~ 40.1) months and the median DFS was 34(range:30.2 ~ 37.8) months.There were statistically significant differences in BCFI and DFS between the two groups(P 0.05).Conclusion It was confirmed that domestic letrozole combined with OFS in the treatment of premenopausal breast cancer patients with high risk of recurrence.The combination of domestic letrozole and OFS could delay the risk of recurrence and metastasis,and had no significant impact on the quality of life of the patients.
