舟山地区女性人乳头状瘤病毒感染高危亚型分布及与CIN2及以上病变的相关性

摘　　要：目的 探讨舟山地区人乳头瘤病毒(HPV)感染基因型别、子宫颈上皮内瘤变(CIN)、子宫颈癌在人群中及年龄段内分布情况,分析宫颈病变与高危型HPV关系.方法 应用《妇幼重大公共卫生服务项目信息直报系统》,对2017-2019年HPV阳性个案进行对比,分析舟山地区HPV型别分布情况及特征.结果 2,544例HPV高危型患者中,检出CIN1占43.25%,CIN2及以上占21.74%.HPV高危型的检出率前五位由高到低依次是HPV52、58、16、51和39.年龄>60岁者检出比例最高(41.82%),50~54岁检出癌症最多(1.46%),45~49岁检出低级别病变最多(21.89%),但40~44岁检出癌前病变最多(8.82%).HPV33、58和16是发生CIN2及以上病变的危险因素,HPV33、58、16、18和CIN2及以上病变呈显著相关性.结论 舟山市女性发生宫颈癌的高发年龄是50~54岁,除外HPV16、18外,HPV33、58阳性者也应给予高度重视.Objective To investigate the distribution of high-risk genotype of human papiDomavirus(HPV),cervical intraepithelial neoplasia(CIN)and cervical cancer in Zhoushan area,and analyze the correlation between cervical lesions and high-risk HPV infection.Methods Using the national major public health project information direct reporting system,the distribution and characteristics of HPV positive cases were compared and analyzed from 2017 to 2019 in Zhoushan.Results 2544 patients with high-risk HPV types were detected CIN1 388 cases(accounted for 43.25%),and CJN2 and above 195 cases(accounted for 21.74%).The top 5 high-risk HPV types was HPV52,followed by HPV58,HPV 16,HPV51 and HPV39.In different age groups,the HPV infection rate of women aged>60 was the highest(41.82%),the cervical carcinoma rate of women aged 50-54 was the highest(1.46%),the low grade lesions rate of women aged 45〜49 was the highest(21.89%),but the precancerous lesions rate of women aged 40~44 was the highest(8.82%).HPV33,58 and 16 were risk factors for CIN2 and above lesions.HPV33,58.16 and 18 were significantly correlated with CIN2 and above lesions.Conclusion The high incidence age of cervical cancer among women is 50〜54 years old in Zhoushan.In addition to HPV 16 and HPV 18,HPV33 and HPV58 positive people should also be paid great attention.
