继发性淋巴水肿的MR淋巴造影表现及临床应用

摘　　要：目的 分析继发性淋巴水肿MR淋巴造影(MRL)表现及临床应用.方法 对11例继发性淋巴水肿患者的19条肢体进行MRL成像,探讨其MRL表现及临床应用.结果 11例中,1例为上肢,10例为下肢;6例出现双侧水肿,2例出现会阴水肿,4例出现单侧肢体水肿;临床分期,19条肢体中,6条为0~Ⅰ期,8条为Ⅱ期,5条为Ⅲ期;有2条肢体淋巴管未见显示,其余17条淋巴管均显示,其中有5条肢体出现淋巴网及侧副管.结论 MRL可直观全面显示淋巴管走形及堵塞情况,在疾病发展的各个时期为临床治疗方式的选择提供依据.Objective To analyze the MR lymphography(MRL)manifestations and clinical application of secondary lymphedema.Methods 19 limbs of 11 patients with secondary lymphedema was performed with MRL to investigate the MRL manifestations and clinical application.Results Among the 11 cases,1 case was upper limb and 10 cases were lower limb,bilateral edema occurred in 6 cases,perineal edema in 2 cases and unilateral limb edema in 4 cases,of the 19 limbs,8 were stage Ⅱ,5 were stage Ⅲ and 6 were stage 0-1,no lymphatic vessels were found on MRL in 2 limbs,and the other 17 lymphatic vessels were shown,lymphatic networks and lateral accessory vessels were found in 5 limbs.Conclusion MRL can directly and comprehensively display the shape and blockage of lymphatic vessels,and provide the evidence for the choice of clinical treatment in each stage of disease development.
