摘　　要：目的分析观察白细胞单采去除术对高白细胞白血病患者的治疗效果。方法搜集昆明医科大学第一附属医院2017年3月—2020年6月接诊的271例高白细胞白血病患者,采用信封法,将患者分为观察组(n=136,单采去除术)和对照组(n=135,羟基脲),观察和比较效果和并发症情况。结果治疗前两组的白细胞、血红蛋白、血小板和PT、APTT、TT、FIB等差异无统计学意义(P>0.05),治疗后白细胞明显改善,差异有统计学意义(P<0.05),观察组治疗后较优,差异有统计学意义(P<0.05),而两组血红蛋白、血小板和凝血功能指标差异无统计学意义(P>0.05),并发症率、早期病死率、治疗持续时间比较显示对照组较差,差异有统计学意义(P<0.05)。结论白细胞单采去除术对高白细胞白血病患者白细胞降低改善效果较好,治疗效率更佳,能在短时间内迅速减轻患者的血液循环负担,能够降低其并发症率和早期病死率,有推广价值。Objective To analyze and observe the therapeutic effect of leukocyte monocytosis in patients with high leukocyte leukemia.Methods A total of 271 patients with hyperleukocytic leukemia admitted to The First Affiliated Hospital of Kunming Medical University from March 2017 to June 2020 were collected.By envelope method,the patients were divided into the observation group(n=136,apheresis)and the control group(n=135,hydroxyurea),and the effects and complications were observed and compared.Results Before treatment,there was no significant difference in white blood cells,hemoglobin,platelets,PT,APTT,TT,FIB,etc.between the two groups(P>0.05).After treatment,white blood cells were significantly improved,and the difference was statistically significant(P<0.05).The observation group after treatment was better,and the difference was statistically significant(P<0.05),while there was no significant difference in hemoglobin,platelet and coagulation function between the two groups(P>0.05).The comparison of complication rate,early mortality rate and treatment duration showed that the control group was worse than the control group,and the differences were statistically significant(P<0.05).Conclusion Leucocyte monocytosis has a better effect on leukocyte improvement in patients with high leukocyte leukemia,and has higher treatment efficiency.It can quickly reduce the burden of blood circulation in patients with high leukocyte leukemia in a short period of time,and can reduce its complication rate and early mortality rate,which is worthy of promotion.
