摘 要:目的探讨T790M突变非小细胞肺癌患者的肺部CT及临床病理特征。方法回顾性分析长沙市中医医院(长沙市第八医院)2020年7月—2022年5月经病理诊断为非小细胞肺癌的65例患者的临床资料并进行相关文献复习。结果非小细胞肺癌T790M突变与患者年龄、性别、是否合并慢阻肺、合并胸膜转移、肝转移及肿瘤分期无明显关系。总生存率在非小细胞肺癌患者中T790M基因突变与非突变患者中差异无统计学意义(P>0.05)。T790M突变的差异与患者组织病理学类型(P=0.009)、吸烟(P<0.05)、脑转移(P<0.01)和骨转移(P<0.01)有关。非小细胞肺癌T790M突变与非突变患者病灶位置差异无统计学意义(P>0.05)。非小细胞肺癌T790M突变与非突变患者在不同类型影像学相关征象方面比较差异无统计学意义(P>0.05)。结论T790M阳性常见于肺腺癌,混合型癌也可见,不吸烟、脑转移、骨转移可以作为T790M阳性的预测因素。Objective To investigate the lung CT and clinicopathological features of patients with T790M mutant non-small cell lung cancer.Methods To retrospective analysis the clinical data of 65 patients in Changsha Hospital of Traditional Chinese Medicine(Changsha Eighth Hospital),who has been diagnosed as NSCLC from July 2020 to May 2022,and to review the literature at the same time.Results The T790M mutation in non-small cell lung cancer was not significantly related to the age,sexual distinction,whether combined with COPD,pleural metastasis,liver metastasis,and tumor stage(P>0.05).There was no significant difference in the overall survival rate between NSCLC patients with T790M mutation or non-mutant(P>0.05).The difference of T790M mutation was related to histopathological types(P=0.009),smoking(P<0.01),brain(P<0.01)and bone metastasis(P<0.01).There was no significant difference between NSCLC patients with T790M mutation or non-mutant in neither the location of lesions or the different types of imaging-related signs(P>0.05).Conclusion T790M mutation is common in lung adenocarcinoma and mixed cancer.Non-smoking,brain metastasis and bone metastasis can be used as predictors of T790M mutation.
