肺移植患者发生毛鞘癌1例

摘　　要：报告1例毛鞘癌。患者男,71岁。患者4年前曾行左肺移植术,3年余前在左颞部发现一绿豆粒大小淡红色丘疹,未诊治。半年前皮损增长速度较快,无红肿、破溃,无瘙痒、疼痛等自觉症状。皮肤科检查:左颞部见一约2.5 cm×2.0 cm大小淡红色斑块,表面略粗糙。皮损组织病理示:肿瘤团块呈侵袭性生长,累及皮脂腺、皮脂腺导管及毛囊,可见细胞异型性及病理性核分裂像。诊断:毛鞘癌。行手术扩大切除+菱形皮瓣成形术。现随访中,未见复发。A case of Trichilemmal carcinoma is reported.A 71-year-old man had undergone left lung transplantation 4 years ago.An asymptomatic light red mass in green-bean size appeared on the left temporal area 3 years ago,without any treatments.The skin lesion grew rapidly since half a year ago.Physical examination showed a reddish plaque sized about 2.5 cm×2.0 cm on the left temporal area with a slightly rough surface.Histopathology showed that the infiltrative tumor mass was involved in the sebaceous glands,sebaceous ducts,and hair follicles.Atypical cells and pathological mitosis were evident.Diagnosis:Trichilemmal carcinoma.Extensive surgical excision and rhomboplasty were performed.No recurrence was observed during follow up.
