摘 要:目的 分析徐州市高碘地区改水后重点人群碘营养状况,为精准防控碘缺乏病提供科学依据。方法 查阅2003和2017年徐州市高碘地区水碘普查资料,根据水碘0-100值分为5层,每层选择1个行政村,检测505名8~10岁儿童尿碘水平及甲状腺情况,调查500名18~40岁育龄妇女尿碘水平。结果 8~10岁儿童甲状腺肿大率为3.96%,5个水碘层次的儿童甲状腺肿大率差别有统计学意义(χ^(2)=19.966,P=0.007),甲状腺结节率为18.22%,5个层次的儿童甲状腺结节率无统计学差异(χ^(2)=1.221,P=0.847);儿童尿碘中位数277.98μg/L,5个水碘层次的儿童尿碘中位数有统计学差异(H=48.801,P=0.001),18~40岁育龄妇女尿碘中位数为296.41μg/L,5个水碘层次的尿碘中位数有统计学差异(H=38.723,P=0.001)。结论 改水后高碘地区中“水碘<100μg/L行政村”8~10周岁儿童和水碘40~80μg/L层次的育龄妇女碘营养状况总体高于适宜状态,居住在水碘80~100μg/L层次的人群其碘营养状况处于过量状态。Objective To analyze the iodine nutrition status of key population in high iodine areas of Xuzhou after water improvement,so as to provide scientific basis for accurate prevention and control of iodine deficiency disease.Methods Based on the data from the 2003 and 2017 water iodine censuses in Xuzhou,administrative villages without iodized salt were selected.To the water iodine 0-100 value was divided into 5 layers,and one administrative village was selected for each level,the urine iodine level and thyroid condition of 505 children aged 8-10 were detected,and the urine iodine level of 500 women aged 18-40 was investigated.Results The goiter rate of children aged 8-10 years was 3.96%.The difference of gotter rate in five water iodine levels was statistically significant(χ~2=19.966,P=0.007).The thyroid nodule rate was 18.22%,there was no significant difference in the thyroid nodule rate among the five levels(χ~2=1.221,P=0.847),the median urinary iodine of children was 277.98μg/L,and the median urinary iodine of the five levels of water iodine was significantly different(H =48.801,P =0.001),the median urinary iodine of 18-40 years old women of childbearing age was 296.41 μg/L,and there was significant difference among the five water iodine levels(H=38.723,P=0.001).Conclusion The iodine nutritional status of children aged 8-10 and women aged 40-80 of water iodine in “water Iodine <100 μg/L administrative village”was higher than that in the suitable condition,the iodine nutritional status of the population living in the level of 80-100 μg/L water iodine was in excess.
