摘 要:目的 研究 3.74 μm激光致新西兰白兔角膜损伤特点和修复过程。方法 发射方式为连续输出的高斯光束,分别照射新西兰白兔双眼,光斑面积 1.89 mm^(2),照射时间 1.0 s,照射剂量约 16、32、64、128 J/cm^(2),于照后即刻、6 h及 1、3、7、14、28、90、180 d大体及裂隙灯观察角膜损伤情况,并用 HE 染色观察角膜组织病理变化。结果 3.74 μm激光致兔眼角膜损伤随激光照射剂量的增加而加重,照射剂量在 16 J/cm^(2)时损伤轻微表浅,32 J/cm^(2)时可伤及角膜内皮层,128 J/cm^(2)以上时可造成角膜穿孔。当角膜损伤只累及浅基质层时可恢复至透明。随损伤深度增加,基质层由纤维结缔组织修复形成角膜瘢痕,且伴不同程度的损伤区变薄,甚至生成圆锥角膜。结论 3.74 μm 激光致角膜损伤及损伤后修复具有剂量依赖性,随着激光照射剂量增大,角膜损伤加重,损伤修复时间延长。Objective To study the characteristics and wound healing of corneal injuries in New Zealand white rabbits after 3.74 μm laser irradiation. Methods The corneas of New Zealand white rabbits were irradiated with 3.74 μm gaussian beam. The spot area was 1.89 mm^(2),the expusure time was 1 s,and the exposure dose was about 16,32,64,128 J/cm^(2).The corneal lesions were observed with slit lamp microscopy immediately and 6 h,1 d,3 d,7 d,14 d,28 d,90 d,and180 d after irradiation. The pathological characteristics of corneal lesions were observed on sections with HE staining.Results The depth of corneal injury caused by 3.74 μm laser increased with the exposure dose. The damage was slight and superficial at the exposure dose of 16 J/cm^(2),the corneal endothelium could be injured at 64 J/cm^(2),and the corneal perforation could be caused at above 128 J/cm^(2). The superficial corneal injury could be restored to transparency. With the increase in the damage depth,the stroma was repaired by fibrous connective tissue to form a corneal scar. The damaged area became thinner and keratoconus could be observed. Conclusion The corneal injury and injury repair process after3.74 μm laser irradiation are dependent on the exposure dose. With the increase in the exposure dose,the corneal injury becomes more serious and wound healing takes a much longer time.
