机械治疗联合抗生素对侵袭性牙周炎患者牙龈下菌斑和血清炎症因子水平的影响

摘　　要：目的探讨机械治疗联合抗生素对侵袭性牙周炎(AgP)患者牙龈下菌斑和血清炎症因子水平的影响。方法选择我院2019年8月至2021年5月收治的100例AgP患者为研究对象,随机将其分为对照组和观察组,各50例。对照组采用洁治、刮治、根面平整术治疗,观察组先采用阿莫西林胶囊和甲硝唑片治疗,治疗1周后,再进行洁治、刮治、根面平整术治疗。比较两组患者治疗前、后的牙周相关指标[出血指数(BI)、牙周探诊深度(PD)、附着丧失(AL)]、致病菌[牙龈卟啉单胞菌(Pg)、伴放线放线杆菌(Aa)、齿垢密螺旋体(Td)]检出情况及炎症因子指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、高敏C反应蛋白(hs-CRP)]水平。结果治疗前,两组患者的BI、PD和AL比较,差异无统计学意义(P>0.05);治疗后,两组患者的BI、PD和AL均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组患者的Pg、Aa和Td表达量比较,差异无统计学意义(P>0.05);治疗后,两组患者的Pg、Aa和Td表达量均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组患者的TNF-α、IL-6、IL-8、hs-CRP水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的TNF-α、IL-6、IL-8、hs-CRP水平均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。结论机械治疗联合抗生素治疗AgP患者可有效地控制龈下菌斑含量,降低PD及TNF-α、IL-6、IL-8、hs-CRP水平,值得在临床中推广实施。Objective To investigate the effects of mechanical therapy combined with antibiotics on subgingival plaque and serum inflammatory factors levels in patients with aggressive periodontitis(AgP).Methods A total of 100 patients with AgP admitted in our hospital from August 2019 to May 2021 were selected as the study objects and randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with scaling,curettage and root planing,the observation group was first treated with amoxicillin capsules and metronidazole tablets,after one week of treatment,and then treated with scaling,curettage and root planing.The periodontal related indexes[bleeding index(BI),probing depth(PD),attachment loss(AL)],detection of pathogenic bacteria[Porphyromonas gingivalis(Pg),Actinobacillus actinomycetemcomitans(Aa),Treponema denticola(Td)]and inflammatory factors indexes[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),high-sensitivity C-reactive protein(hs-CRP)]levels of the two groups were compared before and after treatment.Results Before treatment,there were no significant differences in BI,PD and AL between the two groups(P>0.05);after treatment,the BI,PD and AL of the two groups were lower than those before treatment,those of the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the expressions of Pg,Aa and Td between the two groups(P>0.05);after treatment,the expressions of Pg,Aa and Td of the two groups were lower than those before treatment,those of the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of TNF-α,IL-6,IL-8 and hs-CRP between the two groups(P>0.05);after treatment,the levels of TNF-α,IL-6,IL-8 and hs-CRP of the two groups were lower than those before treatment,those of the observation group were
