桂枝茯苓丸联合GnRH-a治疗围绝经期子宫肌瘤并发异常子宫出血的临床效果分析

摘　　要：目的 探究桂枝茯苓丸联合促性腺激素释放激素激动剂(GnRH-a)治疗围绝经期子宫肌瘤并发异常子宫出血患者的临床效果。方法 86例围绝经期子宫肌瘤并发异常子宫出血患者,以掷骰子法分为参照组和实验组,每组43例。参照组采用GnRH-a治疗,实验组在参照组基础上联合桂枝茯苓丸治疗。对比两组患者临床治疗效果及治疗前后子宫内膜厚度、子宫肌瘤体积、子宫体积、血清性激素[卵泡刺激素(FSH)、黄体生成激素(LH)、雌二醇(E;)、孕酮(P)]水平。结果 实验组患者总有效率为93.02%,高于参照组的76.74%,差异具有统计学意义(P<0.05)。治疗3个月后,实验组子宫内膜厚度(5.28±1.24)mm、子宫肌瘤体积(41.08±6.95)cm^(3)、子宫体积(658.24±86.19)cm^(3)小于参照组的(7.19±1.36)mm、(52.04±6.52)cm^(3)、(744.51±96.37)cm^(3),差异具有统计学意义(P<0.05)。治疗3个月后,实验组患者P、E;、LH、FSH水平明显低于参照组,差异具有统计学意义(P<0.05)。结论 围绝经期子宫肌瘤并发异常子宫出血患者,在临床治疗中联合应用桂枝茯苓丸和GnRH-a的效果显著,能够有效改善病症、控制病情,值得推广应用。Objective To investigate the clinical effect of Guizhi Fuling Pill combined with gonadotropinreleasing hormone agonist (GnRH-a) in the treatment of perimenopausal uterine myoma complicated with abnormal uterine bleeding.Methods A total of 86 patients with perimenopausal uterine fibroids and abnormal uterine bleeding were divided into reference group and experimental group by dice-throwing method,with 43 cases in each group.The reference group was treated with GnRH-a,and the experimental group was treated with Guizhi Fuling Pills on the basis of the reference group.Both groups were compared in terms of clinical effect,endometrial thickness,uterine myoma volume,uterine volume,serum sex hormones[follicle-stimulating hormone(FSH),luteinizing hormone (LH),estradiol (E;),progesterone (P)]level before and after treatment.Results The total effective rate in the experimental group was 93.02%,which was higher than 76.74%in the reference group,and the difference was statistically significant (P<0.05).After 3 months of treatment,the endometrial thickness of the experimental group was (5.28±1.24) mm,the volume of uterine myoma was (41.08±6.95) cm^(3),and the uterine volume was (658.24±86.19) cm^(3),which were less than (7.19±1.36) mm,(52.04±6.52) cm^(3),and (744.51±96.37) cm^(3) of the reference group,and the differences were statistically significant (P<0.05).After 3 months of treatment,the levels of P,E;,LH and FSH in the experimental group were significantly lower than those in the reference group,and the difference was statistically significant (P<0.05).Conclusion The combined application of Guizhi Fuling Pills and GnRH-a has a significant effect in clinical treatment for patients with perimenopausal uterine fibroids and abnormal uterine bleeding,and can effectively improve the symptom and control the disease,which is worthy of popularization and application.
