摘  要:目的:探讨SHOX2和RASSF1A基因甲基化检测在早期肺癌的诊断价值。方法:选择2020年8月-2021年3月期间我院收治肺部小结节患者107例为研究对象,其中经病理活检确认早期肺癌患者72例(肺癌组),肺部良性病变患者35例(对照组)。采集患者肺泡灌洗液(BALF)样本,进行细胞学检查且荧光PCR法测定样本中SHOX2和RASSF1A基因甲基化阳性率,分析甲基化情况及其病理诊断关系。结果:肺癌组和对照组SHOX2基因甲基化阳性率分别为51.4%和2.9%,RASSF1A基因甲基化阳性率分别为61.1%和5.7%,均差异显著(P<0.05),且SHOX2和RASSF1A基因甲基化任意阳性率显著高于细胞学检查阳性率(P<0.05);并联SHOX2和RASSF1A基因甲基化检测早期肺癌的灵敏度和特异性分别为76.4%(55/72)、91.4%(32/35),且在不同病理分型中存在差异,灵敏度在小细胞癌中最高(95.7%,22/23),鳞癌次之(76.5%,13/17),腺癌最低(68.8%,22/32)。结论:SHOX2和RASSF1A基因甲基化任意阳性率在患者BALF中诊断效果良好,有成为早期肺癌临床筛查指标的潜力。Objective:To explore the value of methylated SHOX2 and RASSF1A gene indiagnosisof early lung cancer.Methods:A total of 107 patients with small pulmonary nodules in our hospital from 2020.8 to 2021.3 were divided into lung cancer group(diagnosed with early lung cancer through pathological test)and control group(diagnosed with benign lesion).The BALF sample was collected,then cytologic test was conducted and methylated SHOX2 and RASSF1A gene was detected by PCR.The positive rates were calculated and the relationship between positive rates and pathological type was analyzed.Results:The positive rate of methylated SHOX2 gene in lung cancer group was 51.4%which was significantly higher than that in control group(2.9%);the positive rate of methylated RASSF1A gene in lung cancer group was 61.1%which was significantly higher than that in control group(5.7%)(both P<0.05).The arbitrary positive rate of methylated SHOX2 and RASSF1A gene was significantly higher than the positive rate of cytologic test.The sensibility of methylated SHOX2 and RASSF1A gene parallel detection was 76.4%(55/72)and the specificity was 91.4%(32/35).The sensibilities were different due to pathological types,small cell cancer highest(95.7%,22/23),squamous carcinoma second(76.5%,13/17),adenocarcinoma third(68.8%,22/32).Conclusion:The detection of methylated SHOX2 and RASSF1A gene is helpful todiagnosis of early lung cancer,and has the potential to be the indicator of early lung cancer.
