摘 要:目的分析1例原发性免疫缺陷(Primary immunodeficiency,PID)患者持续排出的Ⅱ型免疫缺陷疫苗衍生脊髓灰质炎(脊灰)病毒(Immunodeficiency vaccine-derived poliovirus,iVDPV)分子病毒学特征。方法对2015年广东省云浮市1例患PID的急性弛缓性麻痹病例连续采集粪便标本并进行脊灰病毒分离,使用逆转录-聚合酶链反应对分离株VP1编码区进行扩增和核苷酸序列测定,分析核苷酸和氨基酸相似性。结果2015年3-10月从该例PID患者分离出13株Ⅱ型脊灰病毒,包括8株iVDPV和5株Sabin疫苗株。13株分离株VP1区与Ⅱ型Sabin疫苗参考株的核苷酸相似性为99.1%-99.6%;随着排毒时间延长,核苷酸和氨基酸变异数量增加,其中I143V和Y185F是所有分离株共享的非同义突变。结论本研究PID患者排出的iVDPV存在共同的初始进化祖先,在患者体内持续复制并发生多种突变。ObjectiveTo analyze molecular virological characteristics of type 2 immunodeficiency vaccinederived polioviruses( i VDPV) excreted from an individual with a primary immunodeficiency( PID).MethodsWe collected fecal samples periodically from an acute flaccid paralysis case with PID in Yunfu city, Guangdong province in 2015 and isolated polioviruses from the samples. We amplified and sequenced the VP1 coding region of isolates using reverse transcription-polymerase chain reaction to determine nucleotide and amino acid similarities.ResultsThirteen strains of poliovirus type 2 were isolated from the PID patient from March to October 2015;8 were i VDPV strains and 5 were Sabin-like strains. Compared with type 2 Sabin vaccine reference strain, nucleotide similarity in the VP1 coding regions of the 13 isolates was 99. 1%-99. 6%. Nucleotide and amino acid mutations increased over time;I143V and Y185F were non-synonymous mutations in all isolates.ConclusionsThe i VDPVs excreted from this PID patient had a common evolutionary ancestor and continuously replicated, accruing mutations.
