摘 要:目的研究初诊多发性骨髓瘤患者阳性病灶的^(18)F-FDG PET/CT代谢参数与细胞遗传学分级的相关性。资料与方法回顾性收集2015年8月—2020年10月于广东省人民医院初诊为多发性骨髓瘤80例患者的^(18)F-FDG PET/CT影像资料,分为高危组54例和标危组26例,以多发性骨髓瘤阳性病灶的最大标准化摄取值(SUVmax)的40%、50%作为阈值,在阳性病灶和参考区(肝右后叶上段)测量代谢参数SUVmax、平均标准化摄取值(SUVmean)、标准化摄取值峰值(SUVpeak)、最大瘦体重标准化摄取值(SULmax)、平均瘦体重标准化摄取值(SULmean)、瘦体重标准化摄取值峰值(SULpeak),计算阳性病灶与肝脏对应代谢参数类型的比值(TLR),得到TLR代谢参数SUVmax TLR、SUVmean TLR、SUVpeak TLR、SULmax TLR、SULmean TLR、SULpeak TLR,并与同期荧光原位杂交检测的细胞遗传学分级进行组间代谢参数差异分析和相关性分析。结果40%、50%作为阈值的SUVmax、SUVmean、SULmean、SUVmax TLR、SUVmean TLR、SULmax TLR、SULmean TLR、SULpeak TLR在高危组与标危组间差异有统计学意义(Z=−3.205~−2.126,P=0.001~0.033)。相关性分析显示,40%、50%SUVmax为阈值的SUVmean TLR、SULmax TLR、SULmean TLR与细胞遗传学分组呈正相关(r_(s)=0.318~0.361,P=0.001~0.004)。结论多发性骨髓瘤的TLR代谢参数稳定性较好,SUVmax TLR、SULmax TLR、SULmean TLR与细胞遗传学分级呈正相关性,且50%为阈值的代谢参数相关性稍优于40%,对无创定量细胞遗传学有重要临床意义。Purpose To investigate the correlation between ^(18)F-FDG PET/CT metabolic parameters of positive lesions and cytogenetic grading in newly diagnosed multiple myeloma.Materials and Methods Retrospective collection of ^(18)F-FDG PET/CT imaging data from 80 cases of multiple myeloma with primary diagnosis of pathology in Guangdong Provincial People's Hospital were performed from August 2015 to October 2020,all patients were divided into 54 cases of high-risk group and 26 cases of standard-risk group,and the metabolic parameters maximum standardized uptake value(SUVmax),SUVmean,SUVpeak,standardized uptake value normalized by lean body mass(SUL)max,SULmean and SULpeak were measured in the positive lesions and the reference area(upper right posterior lobe of the liver)using 40%and 50%of the SUVmax of the positive multiple myeloma lesions as the threshold values.SULmean,SULpeak and the ratio of positive lesions to the corresponding metabolic parameter types in the liver(TLR)were calculated to obtain the TLR metabolic parameters SUVmax TLR,SUVmean TLR,SUVpeak TLR,SULmax TLR,SULmean TLR,SULpeak TLR,and compared with simultaneous fluorescence in situ hybridization assayed cytogenetic grading for intergroup variability analysis of metabolic parameters and correlation analysis.Results The differences of SUVmax,SUVmean,SULmean,SUVmax TLR,SUVmean TLR,SULmax TLR,SULmean TLR,SULpeak TLR between the high-risk and standard-risk groups with 40%and 50%as the threshold values were statistically significant(Z=−3.205-−2.126,P=0.001-0.033).Correlation analysis showed that SUVmean TLR,SULmax TLR,SULmean TLR with 40%and 50%as thresholds were moderately correlated with cytogenetic subgroups,which was statistically significant(r_(s)=0.318-0.361,P=0.001-0.004).Conclusion TLR metabolic parameters of multiple myeloma were stable,SUVmax TLR,SULmax TLR and SULmean TLR showed moderate correlation with cytogenetic grading,and the correlation of metabolic parameters with 50%as the threshold was slightly better than 40%,which is clinically impor
