急性非静脉曲张性上消化道出血短期预后的风险评分比较

摘　　要：目的 比较GBS、原始CCI、更新CCI、AIMS65评分在急性非静脉曲张性上消化道出血(ANVUGIB)的风险分级及短期死亡的预测效果,分析与结果的相关性,筛选适合本地区的预测评分。方法 选取2016年10月至2021年4月首都医科大学密云教学医院住院的260例急性非静脉曲张性上消化道出血患者。按生存情况分为A组(生存组,230例)和B组(死亡组,30例)。观察指标包括年龄、性别、服药情况、合并症、实验室检查等。组间比较采用t检验和χ^(2)检验(pearson卡方、Fisher精确检验)。使用受试者工作特征曲线(ROC)来比较四种评分的预测价值,使用Delong法比较曲线下面积值(AUC)差异。采用Logistic回归分析四种评分及观察指标和结果的相关性。结果 原始CCI、更新CCI、AIMS65评分的风险分级在两组中有差异。更新CCI比原始CCI评分的敏感性和特异性好。更新CCI、AIMS65、原始CCI评分对预测短期死亡有意义。AIMS65、更新CCI评分和结果相关性强。不同评分系统的观察指标和结果的相关性差异大。结论 更新CCI、AIMS65、原始CCI评分对预测短期预后有意义,AIMS65和更新CCI评分和结果的相关性强。Objective Compare the risk classification of GBS,original CCI,updated CCI,and AIMS65 scores in ANVUGIB and the prediction effect of short-term death, analyze the correlation with the results, and provide a basis for improving treatment strategies in the region.Methods Selected 260 ANVUGIB patients who were hospitalized in Miyun teaching hospital of Capital medical university from October 2016 to April 2021.According to their survival status, they were divided into: group A(survival group, 230 cases)and group B(death group, 30 cases).Observation indicators include age, sex, medication status, comorbidities, laboratory tests, etc.The comparison between groups uses t-test and χ^(2) test(pearson chi-square, Fisher’s exact test).ROC is measured to compare the predictive value of the four scores, Use the Delong method to compare the differences.Logistic regression is used to analyze the correlation between the four scores and the observation indicators and the results. Results The original CCI,updated CCI,and AIMS65 scores are different in the two groups.The updated CCI is more sensitive and specific than the original CCI score.Updating CCI,AIMS65,original CCI score is meaningful for predicting short-term death.AIMS65,updated The CCI score and the results are highly correlated.The observation indicators and results of different scoring systems have large correlations. Conclusion Updating CCI,AIMS65,original CCI score is meaningful for predicting short-term prognosis, AIMS65 and updated CCI score are highly correlated with the results.
