双期矫治安氏Ⅱ1错牙合后侧貌改善远期稳定性的临床初探

摘　　要：目的 探讨安式Ⅱ;患者通过Twin-block矫治器双期矫治获得满意的侧貌后,面部软组织侧貌远期的情况。方法 本研究共收集40 例5-9年前处于生长高峰期的下颌后缩安氏Ⅱ;错牙合已进行Twin-block双期矫治结束的患者,采用拍标准面相照,用通过鼻下点的TVL线来测量评价他们与在矫治结束时的侧面相面部侧貌软组织变化情况。测量所得数据资料由SPSS 21.0完成统计学分析。结果 与矫治结束时相比,LLA-TVL,ULA-TVL,LLs-TVL,Pog’-TVL变大,SLs-TVL,Sn-ULA与TVL的夹角,Pog’-LLA与TVL的夹角均变小,特别是Pog’-LLA与TVL的夹角变小更明显,以上结果长期观察变化差值均有显著性差异。结论 处于生长高峰期的下颌后缩安氏Ⅱ;错牙合患者,及时行Twin-block双期矫治后,在取得好的上下咬合关系及坚持口唇肌训练前提下,可获得长期稳定的软组织侧貌,乃至成人以后仍有生长的潜能,面下三分之一的软组织能够进一步改善,使患者的面部侧貌更协调。Objective To investigate the long-term profile of facial soft tissue in patients with Angel’s class Ⅱ~1 malocclusion who obtained satisfactory profile by twin-block appliance.Methods A total of 40 patients with mandibular retraction who were in the peak growth period 5-9 years ago and had undergone Twin-block double-stage orthodontic treatment were collected.TVL line which thorough subnasal point to measure and evaluate the soft tissue changes in their facial profile compared with the face photograph at the end of the treatment.Statistical analysis of the measurements was completed by SPSS 21.0.Results Compared with the end of orthodontic treatment,LLA-TVL,ULA-TVL,LLs-TVL,Pog’-TVL became larger,SLs-TVL,the angle between Sn-ULA and TVL became smaller,and the angle between Pog’-LLA and TVL became smaller more obviously.The above results showed significant differences in long-term observation changes.SLs-TVL became smaller,and LLA-TVL became larger,and there was no significant difference in measurement.Conclusion After timely twin-block double-stage orthodontic treatment and adherence to labial muscle training,the Angel’s class Ⅱ~1 malocclusion patients with mandibular retraction in the peak growth period can obtain a good occlusion and long-term stable soft tissue profile.Even after adulthood,there is still the potential for growth,and the soft tissue of the lower third of the face can be further improved to make the patient’s facial profile more harmonious.
