乳腺浸润性导管癌不同分子亚型与其超声特征的相关性研究

摘　　要：目的分析乳腺浸润性导管癌不同分子亚型与超声特征的相关性。方法选取2018年5月-2021年5月南通瑞慈医院诊治的38例乳腺浸润性导管癌患者为研究对象,分析乳腺癌不同分子亚型间一般情况及超声征象。结果138例浸润性乳腺导管癌患者中Luminal A-like型33例(23.91%)、Luminal B-like型68例(49.27%)、HER-2过表达型17例(12.31%)、三阴型20例(14.49%)。浸润性乳腺导管癌不同亚型间肿块最大直径、是否淋巴结转移比较,差异无统计学意义(P>0.05);浸润性乳腺导管癌不同亚型间组织学分级比较,差异有统计学意义(P<0.05);浸润性乳腺导管癌不同亚型间肿块血流比较,差异无统计学意义(P>0.05);浸润性乳腺导管癌不同亚型间肿块形状、边缘、方向、内部回声、后方回声、钙化比较,差异有统计学意义(P<0.05)。结论Luminal A-like型、Luminal B-like型、三阴型和HER-2过表达型4种分子亚型的乳腺浸润性导管癌都有其特征性超声图像表现,这些超声特征可为乳腺浸润性导管癌的分子亚型诊断、临床治疗及预后提供重要参考。Objective To analyze the correlation between different molecular subtypes of breast invasive ductal carcinoma and ultrasonic characteristics.Methods From May 2018 to May 2021,38 patients with breast invasive ductal carcinoma treated in Nantong Rich Hospital were selected as the study subjects.The general situation and ultrasonic signs of different molecular subtypes of breast cancer were analyzed.Results Among 138 cases of invasive breast ductal carcinoma,33 cases(23.91%)were Luminal A-like,68 cases(49.27%)were Luminal B-like,17cases(12.31%)were HER-2 overexpression,and 20 cases(14.49%)were triple negative.There was no significant difference in the maximum diameter and lymph node metastasis between different subtypes of breast invasive ductal carcinoma(P>0.05).There was significant difference in histological grade among different subtypes of breast invasive ductal carcinoma(P<0.05).There was no significant difference in tumor blood flow among different subtypes of breast invasive ductal carcinoma(P>0.05).There were statistically significant differences in the shape,edge,direction,internal echo,rear echo,and calcification of the masses among different subtypes of breast invasive ductal carcinoma(P<0.05).Conclusion The four molecular subtypes of breast invasive ductal carcinoma,Luminal A-like,Luminal B-like,triple negative and HER-2 overexpression,have their characteristic ultrasound images,which can provide important reference for the diagnosis,clinical treatment and prognosis of breast invasive ductal carcinoma.
